摘 要:目的:探讨枳实薤白桂枝汤对由外源性内皮素-1(endothelin-1,ET-1)诱导家兔冠状动脉痉挛而致心肌缺血的干预作用。方法:将40只家兔随机分为模型组、枳实薤白桂枝汤低剂量组、枳实薤白桂枝汤等效剂量组、枳实薤白桂枝汤高剂量组和法舒地尔组,每组8只。实验开始前5 d对每组家兔进行适应性灌胃,模型组给予等量生理盐水灌胃,枳实薤白桂枝汤低剂量给予0.395 g·kg-1枳实薤白桂枝汤灌胃,枳实薤白桂枝汤等效剂量组给予0.79 g·kg-1枳实薤白桂枝汤灌胃,枳实薤白桂枝汤高剂量组给予1.58 g·kg-1枳实薤白桂枝汤灌胃,法舒地尔组给予法舒地尔悬浊液灌胃。各组家兔灌胃5 d后,经家兔耳缘静脉注射ET-1制备冠脉痉挛(coronary artery spasm,CAS)模型。观察各组家兔药物ET-1注射后5 min、10 min、15 min、30 min心电图T波高度百分比,检测各组家兔血清肌钙蛋白I(cardiac troponin,c Tn I)、血清前列环素(prostacyclin 2,PGI2)、血清血栓烷素(thromboxane 2,TXB2)含量水平变化,并观察心肌组织病理改变。结果:与模型组比较,枳实薤白桂枝汤低剂量、等效剂量、高剂量组及法舒地尔组均可不同程度降低家兔心电图T波增高百分比,降低心肌组织损伤程度,各时间点比较,差异具有统计学意义(P<0.05);药物注射后各时间点PGI2不同程度上升,TXA2、c Tn I下降,差异具有统计学意义(P<0.05);与法舒地尔组比较,枳实薤白桂枝汤高剂量组c Tn I降低,差异具有统计学意义(P<0.05),枳实薤白桂枝汤低剂量组、枳实薤白桂枝汤等效剂量组与其他各相应指标各时间点变化趋势基本相似,差异无统计学意义(P>0.05)。结论:枳实薤白桂枝汤可干预由ET-1诱发家兔冠状动脉痉挛引起心肌缺血变化,对心肌可能具有保护作用。Objective:To explore the Intervention Effect of hnmature Bitter Orange and Chinese Chive and Cinnamon Twig Deeoe- tion(IBOCCCTD)on myocardial ischemia in rabbits indueed by coronary artery spasm due to exogenous endothclin -1 (ET-1). Methods:Forty rabbits were randomly divided into the model group,low-dosage IBOCCCTD group, equivalent dosage IBOCCCTD group,high-dosage IBOCCCTD group,and fasudil group,with 8 cases in each group. In the first five days of the experiment,the rabbits in each group were given adaptive intragastric administration:The ones in the model group were given equivalent mass fraction of 0.9% saline. The ones in the low-dosage IBOCCCTD group were given 0. 395 g ·kg-1 IBOCCCTD for garage. The ones in the equivalent dosage IBOCCCTD group were given 0.79 g ·kg-1 IBOCCCTD for garage. The ones in the high-dosage IBOCCCTD group were given 1.58 g·kg-1 IBOCCCTI) for garage. The ones in the fasudil group were given fasudil suspension for garage. After the rabbits were given gavage for 5 (lays,the coronary spasm (CAS) model was prepared by injecting ET-I into the auricular vein of rabbits. After injecting ET-I for 5,10,15 and 30 minutes respectively, each group's electrocardiogram T wave height pereentage was observed. Each group's serum cardiac troponin I (cTnI) , serum prostaeyclin (PGI2), serum thromboxane (TXB2) level changes were detected, and their pathological changes of myocardial tissue were observed. Results : Compared with the model group,low-dosage IBOCCCTD group, equivalent dosage IBOCCCTD group, high-dosage IBOCCCTD group,and fasudil group,eouht deerease the percentage of T wave of ECG, reduce the degree of myocardial injury, there was significant difference on each time point. After drug injection,in each time point, PGI2 increased in different degree,while TXA2, cTnl decreased, and all the differences were statistically signifieant( P 〈 0.05 ).
