摘　　要：目的了解无细胞百白破联合疫苗(diphtheria tetanus and acellular pertussis combined vaccine,DTaP)预防接种不良反应的发生特征,评价DTaP预防接种的安全性。方法通过中国免疫规划信息管理系统疑似预防接种异常反应信息管理系统,收集河南省2011-2016年DTaP预防接种不良反应个案数据,采用描述流行病学方法进行分析。结果2011-2016河南省共报告DTaP不良反应个案11 050例,年平均报告发生率为28.14/10万剂。其中一般反应10 826例、异常反应224例,报告发生率分别为27.57/10万剂、0.57/10万剂。男女性别比为1.37:1;<2岁儿童占92.04%;接种第4剂次报告发生率最高,47.11/10万剂;报告月份主要集中在7-9月份,占35.10%;报告地区主要集中周口、洛阳、南阳和驻马店等地。不良反应临床诊断主要是发热、红肿、硬结和过敏反应性疾病,异常反应中有过敏性休克、脑脊髓炎等严重病例报告。11 050例不良反应,治愈或好转占99.80%。结论河南省DTaP预防接种安全性良好,不良反应主要是发热、红肿、硬结和过敏性疾病,需关注过敏性休克、脑脊髓炎等严重异常反应的发生。Objective To investigate the characteristics of adverse reactions to diphtheria, tetanus and acellular pertussis com- bined vaccine （DTaP） in Henan Province from 2011 to 2016, and to evaluate the immunization safety related to DTaP. Methods We collected the data about adverse reaction cases following DTaP vaccination in Henan Province from 2011 to 2016 through the Adverse Events Following Immunization Information Management System, the China Information Management System for Immuniza- tion Programming. The descriptive epidemiological method was used for analyzing the data. Results A total of 11,050 adverse reaction cases were reported in Henan Province from 2011 to 2016, and the average reported incidence rate was 28.14 per 100,000 doses. Among the 11,050 cases, 10,826 eases was common adverse reaction, and 224 were rare adverse reaction, with the reported incidence rates being 27.57 per 100,000 doses and 0.57 per 100,000 doses. The ratio of male to female was 1.37： 1. 92. 04% of the cases were children aged 〈2 years. The reported incidence rate of the fourth dose was the highest （47.11 per 100,000 doses）. Most of the cases were reported from April to September, accounting for 35.10%. The cases were mainly concentrated in Zhoukou, Luoy- ang, Nangyang and Zhumadian. The clinical diagnosis of adverse reactions were mainly fever, local swelling, induration and ana- phylactic diseases. Allergic shock and encephalomyelitis were found in the reported rare adverse reaction cases. Among the 11,050 adverse reaction cases, 99.80% were recovered or got better. Conclusions The safety of DTaP immunization in Henan Province is good. The main adverse reactions are fever, local swelling, induration and anaphylactic diseases. However, more attention should be paid to allergic shock, encephalomyelitis and other rare but severe adverse reactions.
