摘  要:目的探讨复发性髌骨脱位患者股骨旋转角度和胫骨旋转角度与髌骨运动轨迹J形征的相关性。方法选取2015年6月至2017年2月北京积水潭医院连续诊断为复发性髌骨脱位患者64例,其中34例存在髌骨运动轨迹J形征的患者作为J形征阳性组,其余30例不存在髌骨运动轨迹J形征的患者作为J形征阴性组。2组患者均接受髋、膝、踝关节CT检查,测量股骨旋转角度(前倾角)及胫骨旋转角度并进行对比分析。结果 64例患者中,男17例,女47例;年龄(21.6±6.7)(16~40)岁。J形征阳性组患者中,男8例,女26例,年龄(21.9±7.8)(12~34)岁。J形征阳性组患者的股骨旋转角度及胫骨旋转角度较J形征阴性组明显增大(P=0.013,0.020)。结论对于复发性髌骨脱位的患者,股骨旋转角度与胫骨旋转角度增大,是导致髌骨运动轨迹出现J形征的风险因素。Objective To investigate the correlations between femoral torsion angle and tibial torsion angle and J-sign of the patella track in patients with recurrent patella dislocation. Methods Sixty-four patients consecutively diagnosed with recurrent patella dislocation from June 2015 to Feburary 2017 were recruited in the present study. Thirty-four patients with J-sign of the patella track were used as the J-sign positive group, the re-maining 30 patients without J-sign of the patella track were used as the J-sign negative group. All of the patients received computed tomography (CT) scan of hip, knee and ankle joints. The femoral torsion angle (antever- sion) and tibial torsion angle were compared between 2 groups respectively. Results There were 17 males and 47 females with an average age of ( 21.6 ±6. 7) years (ranged from 16 to 4 0 yea rs ) . The J-sign positive group (34 patients) included 8 males and 26 females with an average age of ( 2 1 .9 ± 7 . 8) years (ranged from 12 to 30 years). The correlation analysis indicated that femoral torsion angle and tibial torsion angle in the patients with J-sign positive group were significantly higher than that of the J-sign negative group (P = 0 .0 1 3 , 0 .0 2 0 ) . Conclusion Increased femoral torsion angle and tibial torsion angle are predictors of J-sign in patients with re-current patella dislocation.
