绝经早期妇女低剂量激素治疗对体表脂肪、绝经指数和骨质疏松的影响

摘　　要：目的探讨绝经早期妇女低剂量激素治疗对体表脂肪、绝经指数、骨质疏松的影响。方法将312例绝经早期妇女根据用药与剂量方式随机匹配后分为A组(给予戊酸雌二醇片2mg+黄体酮胶囊)、B组(戊酸雌二醇片1mg+黄体酮胶囊)和C组(中药治疗),比较3组治疗有效率、治疗前后体表脂肪、绝经指数、骨质疏松的变化。结果与治疗前比较,A、B和C组各个疗程的胸围、臀围均增加(P<0.05或<0.01),腰围均缩小(P<0.05或<0.01);Kupperman绝经指数均减少(P<0.05或<0.01);骨超声波速度(SOS)值均增加(P<0.05或<0.01);治疗3个疗程后A组的各个指标变化比B、C组明显(P均<0.05)。结论戊酸雌二醇片2mg+黄体酮胶囊组优于戊酸雌二醇片1mg+黄体酮胶囊组和中药组,对绝经早期妇女的体表脂肪分布产生积极作用(胸围增大、腰围变细、臀围增大),影响绝经指数并对骨质疏松有治疗作用。Objective To investigate the effects of low dose hormone therapy on body fat,menopausal index and osteoporosis in early postmenopausal women. Methods Based on medication and dosage,312 early postmenopausal cases were randomly divided into A,B and C group. A group was 2 mg dosage of estradiol valerate tablets and progesterone capsules. B group was 1 mg dosage of estradiol valerate tablets and progesterone capsules. C group was traditional Chinese medicine treatment. The efficiency of treatment on body fat,menopause index and osteoporosis after treatment in 3 groups were compared. Results The indicators about course of treatment changed after therapy. Chest circumference,hip circumference and bone ultrasonic velocity（SOS）increased significantly（P〈0.05 or 0.01）while waist circumference and kupperman menopause index decreased significantly（P〈0.05 or 0.01）in A,B and C 3 groups than those before treatment. It was obvious that every indicator showed change between A,B and C after 3 courses of treatment（P〈0.05）.Conclusion The efficacy of A group（2 mg dosage of Estradiol valerate tablets and progesterone capsules estradiol valerate tablets group）was better than that of B group（1 mg dosage of estradiol valerate tablets and progesterone capsules estradiol valerate tablets）and C group（traditional Chinese medicine group）. It not only had a positive effect physically on fat distribution for early postmenopausal women（chest and hip circumference increases,waist circumference becomes thinner）,but also had therapeutic effect on menopausal index and osteoporosis.
