对维持性血液透析患者家属实施支持性心灵关怀干预的效果研究

摘　　要：目的观察支持性心灵关怀干预对维持性血液透析患者家属提高支持程度和改善心理状况的效果。方法将153名维持性血液透析患者家属采用随机字母表分为观察组(76人)和对照组(77人),对照组家属给予常规的心理咨询和健康教育,观察组实施支持性心灵关怀干预,比较干预前后两组家属领悟社会支持量表、照顾者压力量表、疲劳评定量表、抑郁自评量表、焦虑自评量表分值。结果干预2个月后,观察组家属领悟社会支持评定量表家庭支持维度、朋友支持维度、其他支持维度评分分别为(27.15±3.69)分、(25.83±3.68)分和(26.23±3.57)分,照顾者压力量表评分(52.18±4.40)分,疲劳评定量表评分(74.43±8.61)分,抑郁自评量表评分(56.86±0.74)分,焦虑自评量表评分(58.45±0.42)分,均优于干预前,也优于同期对照组,比较差异均有统计学意义(P<0.05)。结论支持性心灵关怀干预能提高维持性血液透析患者家属的社会支持程度,减轻其照护压力和疲劳感,改善其抑郁、焦虑等负性情绪。Objective To observe the effect of supportive spiritual care intervention on support level and psychological status improvement for family members of maintenance hemodialysis patients.Method Divide 153 family members of maintenance hemodialysis patients into observation group(n=76)and control group(n=77)by random number table.Family members in control group receive routine psychological consultation and health education.Family members in observation group receive supportive spiritual care.Compare scores on perceived social support scale,caregiver strain index,fatigue assessment instrument(FAI),self-rating depression scale(SDS)and self-rating anxiety scale(SAS)in two group before and after intervention.Result 2 months after intervention,score on dimensions of family support,friend support and other support in perceived social support scale in observation group are(27.15±3.69),(25.83±3.68)and(26.23±3.57).Scores on caregiver strain index,FAI,SDS and SAS in observation group are(52.18±4.40),(74.43±8.61),(56.86±0.74)and(58.45±0.42),which are better than scores before intervention and scores in control group after intervention.The difference is significant(P<0.05).Conclusion Supportive spiritual care intervention can improve social support level of family members of maintenance hemodialysis patients,relieve their caring pressure,depression and anxiety.
