摘 要:目的研究临床科室住院医师规范化培训中三级医师分级培训分期考核的效果,为住院医师规范化培训提供方法依据。方法将住培医师分为实验组和对照组,实验组进行本研究分级负责、分期培训考核的培训方法,对照组采用传统培训方法。对两组出科考核专业理论知识、病历质量、临床技能进行对比,并对住培医师在培训过程中培训方法及效果等各方面的满意度进行调查问卷,对数据结果进行分析。结果出科成绩实验组(95.736 8±2.274 2)分,高于对照组的(91.250 0±3.240 4)分,t=49.621;病历质量实验组(92.605 3±4.277 8)分,高于对照组(88.45±3.202 2)分,t=23.744;临床技能实验组(91.4737±3.569 67)分,高于对照组(85.225 0±4.880 6)分,t=41.293,各组间比较,P<0.01,差异有统计学意义。调查问卷对培训教师的满意度、对常见疾病、疑难病例诊治能力提高的满意度、急诊急救能力和医患沟通能力提高的满意度,以及对专业知识掌握的全面系统性等方面新的培训方法均显示了较好的效果,差异具有统计学意义。结论三级医师分级负责分期考核的培训方法明显优于传统培训,可以在临床培训中进一步推广。Objective To study the efect of staged training of three level physicians in the standardized training of residents in clinical departments, so as to provide a basis for standardized training of residents. Methods The resident doctors were divided into the experimental group and the control group. In the experimental group, the training methods were carried out in this study, and the traditional training methods were used in the control group. The examination of professional knowledge, the quality of medical records, clinical skills were compared, the questionnaire was conducted on the satisfaction of the training methods and effects of the trainer in the training process, and the results were analyzed. Results Grades of experimental group (95.736 8±2.274 2), the control group was higher than that of (91.250 0±3.240 4), t=49.621; The medical record quality test group (92.605 3±4.277 8) was higher than that of the control group (88.45±3.202 2), t=23.744; The clinical skill experimental group (91.473 7±3.569 67) was higher than that of the control group (85.225 0±4.880 6), t=41.293, the difference between each group were compared, P 〈 0.01, and the difference was statistically significant. Satisfaction of training teachers, satisfaction of common diseases, difcult cases, improvement of emergency treatment ability and doctor-patient communication ability were investigated by questionnaire. The new training methods, such as the comprehensive and systematic knowledge of professional knowledge, showed good results, and the difference was statistically significant. Conclusion The training method of grade three physician in charge of staging is obviously better than that of traditional training. It can be further promoted in clinical training.
