水飞蓟宾过敏误诊为疱疹性口炎一例报告

摘　　要：水飞蓟宾具有抗氧化、抗肿瘤、抗炎及免疫调节作用,广泛用于各种原因所致肝损害,近年来还发现其有潜在的抗过敏反应。即使较大剂量使用水飞蓟宾仍未发现明显不良反应,尚少见文献报道水飞蓟宾可引起过敏反应。本文中患者因预防结核化疗所致的肝功能损害服用水飞蓟宾,1 d后出现口唇疼痛、糜烂伴咽喉异物感、进食困难,初诊被误诊为"疱疹性口炎",给予抗病毒治疗无效,按过敏反应治疗后痊愈出院。水飞蓟宾作为临床常用的安全有效的预防肝损害的药物,当出现口唇肿胀、糜烂等局部皮肤表现时应警惕过敏可能。It is known that silibinin has antioxidant, anti-tumor, anti-inflammatory and immunomodulatory effects, andwhich is widely used for liver damage caused by a variety of reasons. In recent years, it is found that silibinin has potentialanti-allergic reactions. However, even larger doses of silibinin still show no significant side effects. The rare literaturereports that silibinin can cause allergic reactions. The paper reports a middle-aged patient who orally took silibinin for theprevention of tuberculosis chemotherapy-induced liver damage, and he occurred symptoms of lip pain and anabrosis, foreignbody sensation, and difficulty eating one day after treatment. The patient was misdiagnosed as "vesicular stomatitis" and wastreated by anti-viral therapy. The patient was discharged from the hospital after treating allergic reactions. As a safe andeffective drug for prevention of liver damages in clinic, silibinin should be alert to induce possible allergies when there arelocal skin manifestations such as lip pain and anabrosis.
