双水平呼吸机治疗慢性阻塞性肺疾病急性加重期临床护理疗效观察

摘　　要：目的:探讨双水平呼吸机治疗慢性阻塞性肺疾病急性加重期(AECOPD)临床护理疗效。方法:选取2014年5月~2017年5月本院收治的AECOPD患者80例为研究对象,采用双水平呼吸机治疗,同时进行呼吸机使用前、使用中、使用后护理,观察治疗前后呼吸(RR)、心率(HR)、收缩压(SBP)和舒张压(DBP)变化,动脉血pH、氧分压(PaO_2)、二氧化碳分压(PaCO_2)和氧饱和度(SaO_2)变化,观察辅助呼吸肌评分、急性生理和慢性健康状况Ⅱ(APACHEⅡ)评分和格拉斯哥昏迷评分(GCS评分)变化,统计治疗不良反应和不良情绪发生率。结果:治疗后,患者RR,HR,SBP和DBP显著低于治疗前(P<0.05);治疗后患者pH,PaO_2和SaO_2显著高于治疗前(P<0.05),治疗后PaCO_2显著低于治疗前(P<0.05)。结论:双水平呼吸机治疗AECOPD患者,并施行相应护理干预,可有效降低患者呼吸频率,改善动脉血气指标,减少不良反应和不良情绪的发生。Objective To explore the clinical nursing efficacy of double-level ventilator in the treatment of acute exacerbation of chronic obstructive pulmonary disease（AECOPD）.Methods：A total of 80 patients with AECOPD admitted to our hospital from May 2014 to May 2017 were selected and treated with Bi PAP ventilator.The patients received nursing before use,in use,and after use of ventilator.The changes in respiratory rate（RR）,heart rate（HR）,systolic blood pressure（SBP） and diastolic blood pressure（DBP）,the arterial blood pH,partial pressure of oxygen（PaO_2）,partial pressure of carbon dioxide（PaCO_2） and oxygen saturation（SaO_2） of arterial blood were observed.The changes in auxiliary respiratory muscle score,acute physiological and chronic health evaluation II（APACHE II） score and Glasgow coma score（GCS score） were observed,the incidence of adverse reactions and adverse psychological treatment was recorded.Results：After treatment,the RR,HR,SBP and DBP of patients were significantly lower than before（P〈0.05）;the pH,PaO_2 and SaO_2 were significantly higher than before（P〈0.05）,and the PaCO_2 was significantly lower than before（P〈0.05）.Conclusion：The treatment of BiPAP ventilator in the AECOPD patients combined with corresponding nursing intervention can effectively reduce the respiratory rate,improve arterial blood gas index,and reduce adverse reactions and adverse emotions.
