摘 要:目的探讨丙氨酸氨基转移酶(ALT)及γ-谷氨酰基转移酶(GGT)与2型糖尿病(T2DM)的关系,为T2DM的早期预防提供依据。方法选取参加健康体检并填写调查问卷的2 597名体检者,根据血清ALT或GGT水平分别进行四分位分组(由低到高依次为Q1组、Q2组、Q3组和Q4组),采用Logistic回归分析评估ALT和GGT不同水平与T2DM患病风险的关系。结果随着ALT或GGT水平的升高,体重指数(BMI)、收缩压(SBP)、舒张压(DBP)、血糖(Glu)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)水平及高血压患病率均逐渐升高(P<0.05),高密度脂蛋白胆固醇(HDL-C)均逐渐降低(P<0.001)。在调整了相关混杂因素(年龄、性别、吸烟、饮酒、锻炼、糖尿病家族史、BMI、TG、TC、HDL-C、LDL-C和高血压)后,ALT Q4组的T2DM患病风险是Q1组的1.70倍[比值比(OR)=1.70,95%可信区间(CI)1.05~2.73],GGT Q4组的T2DM患者风险是Q1组的2.38倍(OR=2.38,95%CI 1.77~3.17)。结论血清ALT和GGT水平与T2DM患病风险有关。Objective To investigate the relations of serum alanine aminotransferase(ALT)and gamma-glutamyltransferase(GGT) with type 2 diabetes mellitus(T2DM),and to provide a reference for the early prevention of T2DM. Methods A total of 2 597 healthy subjects, who finished questionnaires,were enrolled. They were classified into 4 groups according to the quartiles of serum ALT and GGT levels(Q1,Q2,Q3 and Q4 groups from low levels to high levels). Logistic regression analysis was used to evaluate the relations of serum ALT and GGT with T2DM. Results Body mass index(BMI),systolic blood pressure(SBP), diastolic blood pressure(DBP), glucose(Glu),total cholesterol(TC), triglyceride(TG),low-density lipoprotein cholesterol(LDL-C)and the prevalence rate of hypertension increased with serum ALT or GGT(P〈0.05), while high-density lipoprotein cholesterol(HDL-C)decreased with serum ALT or GGT(P〈0.001). For serum ALT and GGT, the risks of T2DM in Q4 group had 1.70 times [odds ratio(OR)=1.70,95% confidence interval(CI)1.05-2.73] and 2.38 times(OR=2.38,95%CI 1.77-3.17)higher than those in Q1 group, respectively, after the adjustment of age,sex,smoking,drinking,physical activity,family history of diabetes mellitus,BMI,TG,TC,HDL-C,LDL-C and hypertension. Conclusions Serum ALT and GGT are related with the risk of T2DM.
