超声分型对颈部淋巴结核治疗的指导意义

摘　　要：目的探讨颈部淋巴结核超声分型对临床治疗的指导价值,为临床制订治疗方案提供较可靠的选择依据。方法对145例颈部淋巴结核患者的临床资料进行回顾性分析,按照治疗方案分为内科组和外科组,统计两组间不同超声分型的例数、治愈率、复发率,比较组间治愈率和复发率差别。结果Ⅰ型、Ⅳ型均采取内科治疗,治愈率100%。Ⅱ型78.9%采取内科治疗,治愈率90.0%,复发率10.0%,21.1%采取外科治疗,治愈率100%。Ⅲ型71.2%采取外科治疗,治愈率95.2%,复发率4.8%,28.8%采取内科治疗,治愈率76.5%,复发率23.5%。结论超声分型的Ⅱ型、Ⅲ型内科、外科组治疗方式差异无统计意义,但复发例数外科组较内科治疗组少。超声分型对颈部淋巴结结核治疗方案的选择具有一定的指导价值。Objective To explore guiding significance of ultrasonic classification for treatments of neck lymphatic tuberculosis,and to provide selection basis for clinical therapeutic regimen. Methods The clinical data of 145 patients with neck lymphatic tuberculosis were retrospectively analyzed,accordingto the treatment plan were divided into the internal medicine group and the surgery group,and statistical analyses of the cases number,the cure rate,the recurrence rate of different ultrasonic classificationwere conducted in the two groups,the difference of cure rate and recurrence rate were compared in thetwo groups. Results All the cases of type Ⅰ and Ⅳ were treated with internal medicine and the cure ratewas 100%.Type Ⅱ 78. 9% took internal medicine treatment,the cure rate was 90. 0%,the recurrencerate was 10%,21.1% took surgical treatment,the cure rate was 100%. Type Ⅲ 71.2% adopted surgical treatment,the cure rate was 95. 2%,the recurrence rate was 4. 8%,28. 8% adopted internal medicine treatment,the cure rate was 76. 5%,the recurrence rate was 23. 5%. Conclusion For ultrasonicclassification of type Ⅱand type Ⅲ,internal medicine and surgical treatment had no statistical signifi-cance in the difference,but the case number of recurrence in surgery group is less than that in internalmedicine treatment group.The ultrasonic classification has guiding value in selection of therapeutic regimen for treatments of neck lymphatic tuberculosis.
