内固定联合抗骨质疏松药物治疗老年人桡骨远端骨折的疗效观察

摘　　要：目的观察桡骨远端锁定加压接骨板(LCP)联合抗骨质疏松药物治疗老年骨质疏松性桡骨远端骨折的临床疗效。方法对2010年4月至2015年4月在解放军第457医院骨科进行治疗的90例老年骨质疏松性桡骨远端骨折患者进行回顾分析,其中45例应用LCP+抗骨质疏松药物治疗(试验组),45例应用单纯LCP治疗(对照组)。从骨折愈合时间、骨折部位骨密度值(BMD)和关节功能评价等方面进行比较。结果 90例患者均获随访,平均随访时间16.5个月。试验组骨折愈合时间(10.5±1.4)周,低于对照组(14.3±2.1)周;按照Gartland-Werley腕关节功能评分,试验组优良率为93.3%,对照组88.8%,其疗效比较差异无统计学意义(P>0.05)。试验组BMD术前(0.618±0.105)g/cm^2,术后药物治疗3个月后为(0.680±0.092)g/cm^2,BMD值明显提高,差异有统计学意义(P<0.05);对照组BMD术前(0.621±0.085)g/cm2,术后3个月为(0.627±0.089)g/cm^2,差异无统计学意义(P>0.05)。结论与单纯LCP治疗相比,联合抗骨质疏松药物治疗老年骨质疏松性桡骨远端骨折具有固定可靠、骨折愈合时间短、疗效显著的优点,结合适时积极的康复锻炼是目前治疗该类骨折较为适用的方法。Objective To observe clinical curative effect of radial distal locking compression plate(LCP)combined anti-osteoporosis medicine on treatment of senile osteoporosis distal radius fractures.Methods In April,2010 to April,2015 in the department of orthopedics in the 457 thHospital of PLA, 90 cases of elderly patients with osteoporotic fracture of the distal radius were retrospective analysed,including 45 cases treated with LCP + anti-osteoporosis medicine(experimental group), 45 cases treated with LCP alone(control group). The fracture healing time, the fracture bone mineral density(BMD)values and the joint function assessment were comparatively analysed. Results 90 patients allhave been followed up, with an average follow-up time of 16. 5 months. The fracture healing time of theexperimental group(10. 5 ± 1. 4)weeks was less than that of the control group(14. 3 ± 2. 1)weeks;according to the Gartland-Werley wrist joint function score, the excellent rate of the experimentalgroup was 93. 3% and that of the control group was 88. 8%, the efficacy differences were not statisticallysignificant( P > 0. 05). Preoperative BMD in the experimental group was(0. 618+0. 105)g/cm2, post-operative medication for three months was(0. 680+0. 092)g/cm2, BMD values increased significantly,the differences were statistically significant( P < 0. 05);preoperative BMD in the control group was(0. 621+ 0. 085)g/cm2, postoperative three months was(0. 627+0. 089)g/cm2, the differences were notstatistically significant( P > 0. 05). Conclusion Compared with pure LCP treatment, the LCP com-bined anti-osteoporosis medicine in treatment of senile osteoporotic distal radius fractures has reliable fixation, the fracture healing time is short, the curative effect is distinct, the method combined with posi-tive rehabilitation exercise timely is applicable for treatment of such fractures.
