三腔导尿管球囊、COOK球囊、欣普贝生三种不同方法对孕晚期促宫颈成熟与引产效果观察

摘　　要：目的:对三腔导尿管球囊、COOK球囊、欣普贝生三种不同引产方法用于足月妊娠促宫颈成熟及引产效果、安全性、有效性进行比较。方法:将本院产科及妇保院区产科就诊的120例有引产指征的孕妇,分为三腔导尿管、欣普贝生、COOK球囊三组,回顾性分析三组促宫颈成熟效果、分娩方式、产程时间及分娩并发症情况。结果:1.导尿管球囊组促宫颈成熟总有效率92.5%,COOK球囊组97.5%,欣普贝生组100%;三腔导尿管组与COOK球囊组、欣普贝生组的总有效率比较差异无统计学意义(P>0.05)。2.导尿管球囊组剖宫产率15%,COOK球囊组12.5%,欣普贝生组30%;三腔导尿管与COOK球囊组比较差异无统计学意义(P>0.05),三腔导尿管与欣普贝生组比较差异有统计学意义(P<0.05)。3.导尿管球囊组方面并发症7.5%,COOK球囊组15%,欣普贝生组52.5%。三腔导尿管与COOK球囊组差异无统计学意义(P>0.05),三腔导尿管与欣普贝生组差异有统计学意义(P<0.05)。结论:三腔导尿管球囊联合破膜加小剂量缩宫素促宫颈成熟及引产效果明显,分娩并发症低,孕妇及医生应用的依从性高,故值得临床推广及应用。Objective： To study the induction effect,safety and effectiveness in three different methods of three cavity catheter balloon,COOK balloon and propess. Methods： 120 cases of pregnant women with induced labor indications treated in obstetrics department of our hospital and maternity clinic,were divided into three groups： three cavity catheter balloon group,COOK balloon group and propess group. Then these cases were retrospectively analyzed the effect in promoting cervical maturity,delivery mode,labor and childbirth complications. Results：（1） The effective rate of promoting cervical maturity was 92. 5% in three cavity catheter balloon group,97. 5%in COOK balloon group,and 100% in propess group,respectively. Three cavity catheter group compared with COOK balloon group and propess group,total effective rate is no statistically significant difference（P 0. 05）.（2） The cesarean section rate was 15% in three cavity catheter balloon group,12. 5% in COOK balloon group,and 30% in propess group. There was no statistically significant difference between the three lumen catheters and the COOK group（P 0. 05）. The difference between the three lumen catheter and the hsin pubersheng group was statistically significant（P 0. 05）.（3） Complication rate was 7. 5% in three cavity catheter balloon group,15% in COOK balloon group,and 52. 5% in propess group. There was no statistically significant difference between the three lumen catheters and the COOK balloon group（P 0. 05）,and the difference between the three lumen catheters and the simpabin group was statistically significant（P 0. 05）. Conclusion： Three cavity catheter balloon combined with broken membrane with small dose of oxytocin had obvious effects in promoting cervical maturity and induced labor with low childbirth complications,high compliance in pregnant women and doctors,which is worth to clinical promotion and application.
