摘 要:目的:探讨多层螺旋CT、超声及腹部X线检查在腹部闭合性创伤诊断中的应用价值。方法:选择腹部闭合性创伤患者108例,术前均分别采用多层螺旋CT、超声及腹部X线对腹部闭合性创伤做出初步诊断,并进行开腹探查术明确诊断,对比分析各种检查诊断敏感性、特异性、阳性及阴性预测值、准确性。结果:多层螺旋CT检查在腹部闭合性创伤诊断中灵敏度、特异度、阳性及阴性预测值、准确性均高于超声及腹部X线检查,差异均有统计学意义(P<0.05);超声检查及腹部X线检查上述各项诊断指标比较差异无统计学意义(P>0.05)。结论:CT在腹部闭合性创伤诊断中具有较高的灵敏度、阳性预测值及准确性,与腹部超声及X线检查比较有明显优势。Objective :To evaluate the value of multi-slice spiral CT, uhrasonography and abdominal X-ray examination in the diagnosis of abdominal closed trauma. Methods: 108 case of abdominal closed trauma patients were selected. The patients were diagnosed with multi-slice spiral CT, uhrasonography and abdominal X-ray respectively. The final diagnosis of abdominal closed trauma was confirmed by laparotomy. The sensitivity, specificity, positive and negative predictive value and accuracy of patients were analyzed and compared. Results :The sensitivity, specificity, positive and negative predictive value and accuracy of multi-slice spiral CT in abdomina] closed trauma were higher than those in ultrasound and abdominal X-ray with statistical significance (P 〈 0.05 ). Uhrasonography and abdominal X-ray examination in the above diagnostic indicators showed no statistical difference (P〉0.05). Conclusion:In the diagnosis of closed abdominal trauma, CT has a high sensitivity, positive predictive value and accuracy, respectively, with the obvious advantages as compared to abdominal ultrasound and X-ray examination.
