摘　　要：目的:探讨多层螺旋CT、超声及腹部X线检查在腹部闭合性创伤诊断中的应用价值。方法:选择腹部闭合性创伤患者108例,术前均分别采用多层螺旋CT、超声及腹部X线对腹部闭合性创伤做出初步诊断,并进行开腹探查术明确诊断,对比分析各种检查诊断敏感性、特异性、阳性及阴性预测值、准确性。结果:多层螺旋CT检查在腹部闭合性创伤诊断中灵敏度、特异度、阳性及阴性预测值、准确性均高于超声及腹部X线检查,差异均有统计学意义(P<0.05);超声检查及腹部X线检查上述各项诊断指标比较差异无统计学意义(P>0.05)。结论:CT在腹部闭合性创伤诊断中具有较高的灵敏度、阳性预测值及准确性,与腹部超声及X线检查比较有明显优势。Objective ：To evaluate the value of multi-slice spiral CT, uhrasonography and abdominal X-ray examination in the diagnosis of abdominal closed trauma. Methods： 108 case of abdominal closed trauma patients were selected. The patients were diagnosed with multi-slice spiral CT, uhrasonography and abdominal X-ray respectively. The final diagnosis of abdominal closed trauma was confirmed by laparotomy. The sensitivity, specificity, positive and negative predictive value and accuracy of patients were analyzed and compared. Results ：The sensitivity, specificity, positive and negative predictive value and accuracy of multi-slice spiral CT in abdomina] closed trauma were higher than those in ultrasound and abdominal X-ray with statistical significance （P 〈 0.05 ）. Uhrasonography and abdominal X-ray examination in the above diagnostic indicators showed no statistical difference （P〉0.05）. Conclusion：In the diagnosis of closed abdominal trauma, CT has a high sensitivity, positive predictive value and accuracy, respectively, with the obvious advantages as compared to abdominal ultrasound and X-ray examination.
