大连地区女性人乳头瘤病毒感染情况

摘　　要：目的研究大连地区女性宫颈细胞中人乳头瘤病毒(HPV)感染情况,为该地区防治HPV感染和宫颈癌预防提供科学、可靠的临床数据。方法回顾性分析2014-08/2016-05在解放军第210医院就诊的女性患者3 476例,采用基因芯片法检测26种HPV基因亚型,统计分析HPV感染率及基因亚型分布情况。结果 3 476例女性患者中,HPV阳性感染732例,总感染率21.06%(732/3 476)。全部26种基因亚型均被检出,全部基因亚型共检出943次,其中高危基因亚型检出713次(75.61%),低危基因亚型检出230次(24.39%)。高危基因亚型检出率显著高于低危基因亚型,差异有统计学意义(P<0.05)。HPV感染率最高的高危基因亚型是HPV16,占HPV感染病例的17.49%(128/732),与其他组比较差异均有统计学意义(P<0.05)。感染率最高的低危基因亚型是HPV54,占感染HPV病例的8.06%(59/732)。≤30岁年龄组HPV感染率最高(26.63%),与其他年龄组比较差异均有统计学意义(P<0.05);其他组间比较差异无统计学意义。580例为HPV单一基因亚型感染,感染率为79.24%(580/732);152例为HPV多重基因亚型感染,感染率为20.76%(152/732),最多同时有5种HPV基因亚型感染。HPV单一感染发生率显著高于HPV多重感染,差异有统计学意义(P<0.05)。在HPV多重感染中,合并HPV16基因亚型感染最常见,感染率为25.66%(39/152)。HPV多重感染患者在41~50年龄组占比最高(为26.79%),与51~60年龄组比较差异有统计学意义(P<0.05);与其他年龄组比较差异未发现有统计学意义。结论大连地区女性HPV感染以HPV16型代表高危亚型为主,同时存在HPV单一感染和混合感染,HPV感染基因亚型有显著的人群特点及地域差异。Objective To study the infection of human papilloma virus （HPV） in cervical cells of female people in Dalian area, and to provide scientific and reliable clinical data for the prevention and treatment of HPV infection and cervical cancer. Methods A retrospective analysis was made from August 2014 to July 2016; detection of HPV gene subtype was performed in 3 476 women. HPV detection rates and infection subtypes in women of different ages were compared. Results HPV infection rate was 21.06% （732/3 476）. All of the 26 kinds of gene subtypes were detected, gene subtypes were detected 943 times in total. High - risk gene subtypes were detected 713 times, accounting for 75.61% ; low -risk gene subtypes were detected 230 times, accounting for 24. 39%. The detection rate of high risk genes was significantly higher than that of low risk genes, and the difference was statistically significant （P 〈 0.05 ）. The high - risk gene subtype with the highest risk of HPV infection rate was HPV16, accounting for 17.49% （128/732） of HPV infection cases, and there were significant differences between the other groups （P 〈 0. 05 ）. The low risk gene subtype with the highest infection rate was the HPV54, accounting for 8.06% of HPV cases （59/732）. The highest infection rate of the HPV was 26.61% in the ~〈30 years old age group, and compared with other age groups, the infection rate was statistically significantly different, but the infection rates in the other age groups showed no statistically significant difference. In 580 cases with single HPV subtype infection, the infection rate was 79.24% （580/ 732）. In 152 cases with multiple HPV subtype infection, the infection rate was 20. 76% （152/732）. The maximum number of multiple HPV subtypes in one case was 5. The single infection rate was significantly higher than that of multiple infection, and the difference was statistically significant （P 〈 0. 05 ）. In multiple infections, the most common subtype was HPV16, and the infection rate was 25.66
