摘  要:目的探究感染控制前后先天性心脏病患儿肺部感染超声心动图的表现差异。方法选取2012年3月-2016年3月济南市中心医院收治的324例先天性心脏病并发肺部感染患儿为研究对象,采用超声心动图检查,比较感染控制前后患儿三尖瓣反流程度及反流速度、肺动脉收缩压/主动脉收缩压比值。结果肺部感染控制前,部分患儿心脏血流存在由右向左分流以及双向分流,肺部感染控制后,所有患儿心脏血流均为由左向右分流(P<0.05);患儿肺部感染控制前重度反流168例,肺部感染控制后重度反流60例,肺部感染控制后先天性心脏病患儿三尖瓣反流程度明显优于控制前(P<0.05);患儿肺部感染前三尖瓣反流速度为(215.53±13.27)cm/s,肺部感染控制后三尖瓣反流速度为(168.59±12.18)cm/s,感染控制后反流速度明显减慢(P<0.05);肺部感染控制前先天性心脏病患儿心脏血液分流速度为(154.86±14.18)cm/s,感染控制后为(174.35±15.34)cm/s,感染控制后心脏血液分流速度明显提高(P<0.05);先天性心脏病患儿进行肺部感染控制后肺动脉收缩压/主动脉收缩压比值0.49±0.15,低于肺部感染控制前(P<0.05)。结论三尖瓣反流程度、反流速度以及肺动脉收缩压/主动脉收缩压比值等指标反映了先天性心脏病患儿肺部感染控制的效果,在对先天性心脏病患儿肺部感染进行超声心动图检测时要注意不同类型先天性心脏病患儿心脏血流流向和分流速度。OBJECTIVE To explore the features of echocardiography of congenital heart disease children complicated with pulmonary infections before and after the control of the infections.METHODS A total of 324 congenital heart disease children complicated with pulmonary infections who were treated in Ji′nan Central Hospital from Mar 2012 to Mar 2016 were recruited as the study objects and received the echocardiography.The tricuspid regurgitation severity and velocity,ratio of pulmonary arterial systolic pressure to aortic systolic pressure were observed and compared before and after the control of infections.RESULTS The cardiac blood flow of some children was right-to-left shunt and bidirectional shunting before the control of the pulmonary infections;the cardiac blood flow of all the children was left-to-right shunt after the control of the pulmonary infections(P〈0.05).There were 168 children who had severe regurgitation before the control of the pulmonary infections and 60 children with severe regurgitation after the control of the pulmonary infections,and the severity of tricuspid regurgitation of the children with congenital heart disease was significantly better after the control of the pulmonary infections than before the control of the infection(P〈0.05).The tricuspid regurgitation velocity of the children was(168.59±12.18)cm/s after the control of the pulmonary infection,significantly slower than(215.53±13.27)cm/s before the control of the pulmonary infection(P〈0.05).The cardiac blood shunting velocity of the children with congenital heart disease was(174.35±15.34)cm/s after the control of the pulmonary infection,significantly faster than(154.86±14.18)cm/s before the control of the pulmonary infection(P〈0.05).The ratio of pulmonary arterial systolic pressure to aortic systolic pressure of the children with congenital heart disease was 0.49±0.15 after the control of the pulmonary infection,significantly lower than that before the control of the pulmonary infection(P〈0
