循证护理在白血病患者化疗期间感染护理中的应用效果及对患者生活质量的影响

摘　　要：目的分析循证护理（EBN）在白血病患者化疗期间感染护理中的应用效果及对患者生活质量的影响。方法选取2015年10月-2016年10月期间在医院接受化学治疗的白血病患者80例,按照随机、对照、双盲试验原则分为试验组和对照组,每组各40例;对照组患者给予常规护理,试验组患者接受循证护理;比较两组患者感染率及生活质量评分,并分析两组患者的病原菌检出情况。结果试验组患者感染率为25.00%（10/40）,明显低于对照组感染率57.50%（23/40）（P〈0.05）;循证护理实施前两组患者生活质量评分比较,差异无统计学意义,实施后生活质量评分均提高,且试验组患者生活质量总分、环境领域及社会关系评分均高于对照组（P〈0.05）;试验组10例感染患者共分离出菌株21株,对照组23例感染患者分离出49株,且检出病原菌均以革兰阴性菌为主。结论对白血病化疗患者应用循证护理,有利于降低感染率,改善生活质量,可在临床进行推广和应用。OBJECTIVE To observe the effect of evidence-based nursing（EBN）on leukemia patients with infections during chemotherapy and the impact on quality of life of the patients.METHODS A total of 80 leukemia patients who received chemotherapy in the hospital from Oct 2015 to Oct 2016 were enrolled in the study and divided into the experimental group and the control group according to the principles of randomized,controlled,and double blind trial,with 40 cases in each group.The control group was given conventional nursing,while the observation group was given the evidence-based nursing.The infection rates and scores of quality of life were compared between the two groups of patients,and the distribution of pathogens isolated from the two groups of patients was observed.RESULTS The infection rate of the experimental group was 25.00%（10/40）,significantly lower than57.50%（23/40）of the control group（P〈0.05）.There was no significant difference in the score of quality of life between the two groups of patients before the evidence-based nursing was carried out,the score of quality of life of the two groups was improved after it was carried out,and the total score of quality of life and the scores of environmental field and social relations were significantly higher in the experimental group than those in the control group（P〈0.05）.Totally 21 strains of pathogens were isolated from 10 patients with infections in the experimental group,49 strains of pathogens were isolated from 23 patients with infections in the control group,and the gramnegative bacteria are dominant among the isolated pathogens.CONCLUSIONThe evidence-based nursing may help to reduce the incidence of infection in the leukemia patients during chemotherapy and improve the quality of life,and it is worthy to be promoted in the hospital.
