须癣毛癣菌对3种外用抗真菌药物敏感性的研究

摘　　要：目的通过对3种外用抗真菌药物(2%酮康唑乳膏,盐酸布替萘芬乳膏,曲安奈德益康唑乳膏)体外抗真菌活性和药物敏感性进行比较,为临床的合理用药提供参考。方法选用临床分离须癣毛癣菌15株及其标准株1株,用琼脂扩散法作药敏试验,在琼脂的含菌平板打孔后分别加入上述3种药物,培养7 d后测定各含药孔周围的抑菌圈直径。结果 3种外用药物对须癣毛癣菌其均形成抑菌圈,2%酮康唑乳抑菌圈直径均数15.94 mm,盐酸布替萘芬乳膏抑菌圈直径均数为68.39 mm,曲安奈德益康唑乳膏抑菌圈直径均数为62.32 mm。而任意2组的抑菌圈均数比较,其差异均有统计学意义(P<0.001),2%酮康唑乳膏和盐酸布替萘芬乳膏临床菌株与标准菌株抑菌圈均数比较,其差异均有统计学意义(P<0.001)。结论对于须癣毛癣菌抗真菌活性而言,盐酸布替萘芬乳膏优于曲安奈德益康唑乳膏,曲安奈德益康唑乳膏优于酮康唑乳膏,须癣毛癣菌临床菌株对于酮康唑乳膏与盐酸布替萘芬乳膏不排除耐药情况的产生。Objective This study on three kinds of topical antifungal drugs（2% ketoconazole cream,butenafine hydrochloride cream,triamcinlone acetonide and econazoie nitrate cream） the comparison of in vitro antifungal activity and drug sensitivity,provide reference for clinical rational drug use.Methods The agar diffusion method was used to judge drug sensitivity.Select 15 strains of clinical isolates t.mentagrophytes 1 strain and standard strain,punching AGAR plate containing bacteria,respectively,to join the three drugs,drug cultivation,7 days to determine the bacteriostatic circle diameter.Results Three kinds of drugs is the formation of bacteriostatic ring for t.mentagrophytes.2% ketoconazole cream bacteriostatic circle diameter of 15.94 mm on average,butenafine hydrochloride cream round cloth paste the average diameter of 68.39 mm.triamcinlone acetonide and econazoie nitrate cream average bacteriostatic circle diameter of 62.32 mm.Any bacteriostatic annulus compared two groups of drug,the difference statistically significant（P〈0.001）.Compared with the standard strains,2% ketoconazole cream and butenafine hydrochloride cream group significant difference（P〈0.001）.Conclusion The antifungal activity of t.mentagrophytes,cloth butenafine hydrochloride cream is better than that of triamcinlone acetonide and econazoie nitrate cream,triamcinlone acetonide and econazoie nitrate cream is better than that of 2% ketoconazole cream compared with the corresponding standard bacteria,2% ketoconazole cream and butenafine hydrochloride cream was statistically significant,does not exclude the resistance.
