急诊患者入院前急救探析

摘　　要：目的:通过对北京安贞医院8年余院前急救交接单回顾性分析,研究院前急救患者的状况,为加强院前急救提供数据支持。方法:自2008年至2016年,从院前急救患者交接单中抽样进行统计分析,研究院前急救患者人群特征、发病地点分布、病种、院前医疗救治和急救机构。结果:8年余院前急救的患者数量增加近7倍以上,公众对急救公共卫生需求有明显增加;在性别特征方面,男性数量高于女性,约1.5∶1;在年龄特征方面,院前急救患者的高发人群主要是中老年人群体,尤其是低龄老年和中龄老年群体;在地点特征方面,院前急救患者的发病地点分布很不均衡,家庭是发病最主要的场所,约占2/3;院前急救患者疾病占比依次为心血管病、脑血管病、消化系统、呼吸疾病、外伤、其他、妇产科、昏迷和死亡等;在120与999急救选择上,民众选择120多于999。结论:加强急诊建设,开放急救绿色通道;对心血管病史老年人群应加强二级预防;家庭急救是重要的。Objective： This study takes Beijing Anzhen hospital as an example to conduct an epidemiological analysis on patients in need of pre-hospital first aid,in order to provide data support to pre-hospital emergency care. Methods： A statistical analysis was performed during this study by sampling data from pre-hospital emergency records kept by a comprehensive hospital in Beijing between 2008 and 2016. Results： During these nine years,the number of patients in need of pre-hospital first aid increased seven times,meaning that public demand for emergency public health has been rising. In terms of gender characteristics,the ratio of men and women is 1. 5∶ 1. In terms of age characteristics,the patients are mainly the elderly,especially the elderly aged 60 ＋ and younger elderly group. In terms of site features,the patients calling for pre-hospital aid are mainly at in home,accounting for 2/3 of the total. The disease sequences are cardiovascular diseases,digestive system problems,respiratory diseases,traumas,obstetric and gynecological conditions,comas and etc. In choosing the emergency hotlines of 120 and 999 to call for first aid,people prefer to call 120 for help. Conclusion： To enhance first aid construction,open first aid fast track; establish two-stage prevention for elderly people with cardiovascular diseases; The importance of home first aid.
