宁夏固原地区182例房颤患者抗凝治疗现状及影响因素分析

摘　　要：目的了解宁夏固原地区房颤患者规范化抗凝治疗现状,并分析影响规范化抗凝治疗的因素。方法选取住院治疗的房颤患者182例,并进行CHA_2DS_2-VASc评分,回顾脑卒中高危患者(CHA_2DS_2-VASc>2分)住院期间抗凝治疗方案,并分析影响其规范化抗凝治疗的因素。结果住院的房颤患者中以CHA_2DS_2VASc>2分者居多数;患者年龄越大,服用华法林规范化抗凝治疗依从性越差,不能配合检测INR是影响华法林抗凝治疗的主要因素。结论宁夏固原地区脑卒中高危房颤患者规范化抗凝治疗现状不容乐观,应提高医务人员对脑卒中高危房颤患者的识别及加强对脑卒中高危房颤患者健康教育需要的重视程度。Objective To understand the present situation of standardized anticoagulant therapy for patients with atrial fibrillation in Guyuan area of Ningxia and analyze the factors affecting of the normalization of anticoagulant therapy in order to provide guidance for standardized anticoagulant therapy for patients with atrial fibrillation. Methods 182 patients with atrial fibrillation treated in the people＇s hospital of Guyuan were selected from October 2014 to September 2016. CHA2 DS2 - VASc score were been carried out. The patients with high risk of stroke （ CHA2 DS2 - VASc 〉 2） was reviewed the anticoagulant therapy during hospitalization in order to analyze the factors influencing of the normalization of anticoagulant therapy. Results The patients with atrial fibrillation in the hospital who have CHA2 DS2 - VASc 〉 2 points is the most. The older the patients were, the worse the compliance of the normalization of anticoagulant therapy with warfarin was. INR is the main factor affecting of the anticoagulant therapy with warfarin. Conclusion The present situation of standardized anticoagulant therapy for patients with high risk atrial fibrillation is not optimistic. It is important to improve the identification of patients with high risk atrial fibrillation of stroke and Strengthen the health education for patients with high risk atrial fibrillation.
