摘 要:目的探讨不同程度的溶血、脂血血清样本对生化检测项目医学决定水平结果的影响。方法选择血清中钾(K)、糖(GLU)、白蛋白(ALB)、天门冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、尿素(UREA)、肌酐(CREA)项目的近似医学决定水平浓度的样本各30份,每管中加入6个不同浓度的溶血、脂血干扰物上机检测,统计分析检测结果的差异。结果 K、AST、ALT(溶血指数+^++++)、ALB(溶血指数++++时),与对照组相比差异有统计学意义(P<0.05);UREA、CREA、GLU(溶血指数+^++++)、ALB(溶血指数+^+++时),与对照组相比差异无统计学意义(P>0.05)。AST、ALT、GLU(2.8 mmol/L)(脂血指数>++时),与对照组相比差异有统计学意义(P<0.05);K、UREA、CREA、ALB、GLU(7.0 mmol/L、10.0 mmol/L)(脂血指数+^++++)、AST、ALT(脂血指数+^++时),与对照组相比差异无统计学意义(P>0.05)。结论溶血程度对K、AST、ALT及ALB产生不同影响,尤其对低中浓度医学决定水平限值的临床意义影响更大。脂血对AST、ALT 3个浓度及GLU(2.8 mmol/L)的浓度均失去了医学决定水平的临床意义。Objective To investigate the effects of different levels of hemolysis and lipemia on the serum test commonly used in biochemical markers of the medical decision level. Methods Serum samples of similar medical decision level concentration of K, GLU, ALB, AST, ALT, UREA, CREA and other items were selected and each of the medical decision levels of 30 samples, each tube were added six different concentrations of hemolysis, lipemia distractors and used the machine to detect. Using SPSS16.0 software to analyze the difference of the test results. Results The effect of hemolysis : K, AST, ALT ( hemolysis index + ~ + + + + ), ALB ( hemolysis in- dex + + + + ,P 〈 0. 05 ), the difference was statistically significant and interfere the different medical decision levels ; UREA, CREA, GLU(hemolysis index + - + + + + ) ,ALB (hemolysis index + - + + + ,P 〉0.05), the difference are meaningless and have no effect on different medical decision levels. The effect of lipemia: AST, ALT, GLU ( 2.8mmol/L, lipids index 〉 + + , P 〈 0.05 ), the difference was statistically significant and interfere the different medical decision levels;K, UREA, CREA, ALB, GLU (7.0 mmol / L, 10.0 mmol/L,Lipid index + - + + + + ), AST, ALT ( Lipid index + - + + , P 〉 0. 05 ), The difference are meaningless and have no effects on different medical decision levels. Conclusion At the time of hemolysis index 〉 +, K, AST, ALT and hemolysis index 〉 + + + , ALB and lipid index 〉 + + ,the three medical decision levels of AST, .ALT and GLU (2.8 mmol/L) medical decision level have lost the clinical significance of the medical decision levels, so such samples should not detect the items above. Hemolysis on UREA, CREA, GLU results were no interference and lipemia on the K, UREA, CREA, ALB results also were no interference.
