超声造影与SPECT肾动态显像在原发性高血压早期肾损伤中的对比研究

摘　　要：目的 :对比分析超声造影(CEUS)与SPECT肾动态显像对原发性高血压早期肾损伤的评估,明确CEUS在评估高血压患者早期肾损伤中的临床应用价值。方法:收集40例高血压患者作为病例组,30例健康志愿者作为正常对照组,所有患者的血肌酐及尿素氮水平均在正常范围内,均随机分为C组(行CEUS检查)和S组(行SPECT肾动态显像检查),分别对其肾功能进行定量分析。结果:CEUS检查的患者中,与正常对照组相比,高血压早期肾损伤组时间-强度曲线(TIC)形态较正常对照组圆缓,曲线上升、下降均较对照组缓慢,曲线下面积(AUC)增大,达峰时间(TTP)升高,峰值强度(PI)降低(P<0.05),而平均通过时间(MTT)延长,但差异没有统计学意义(P>0.05);而S组中的结果显示,病例组与正常对照组相比,分肾的GFR有一定下降趋势,但差异没有统计学意义(P>0.05)。结论:与SPECT肾动态显像相比,CEUS在评估高血压早期肾损伤时更敏感,能够更早地为临床提供简单、安全、实时、灵敏的高血压早期肾损伤监测指标。Objective： To compare the value of renal dynamic imaging and contrast-enhanced ultrasound in assessment of early renal damage in patients with hypertension. Methods： Forty cases of hypertension were included in our study, and 30 healthy volunteers were enrolled as normal control group. Each patient had normal results in blood serum urea nitrogen（BUN） and serum creatinine（SCr） tests. Hypertension patients and control group were randomly divided into C group（with CEUS） and S group （with SPECT）, and the renal function were quantitatively analyzed. Results： In CEUS group, compared with normal control, the curves of hypertension patients were more blunt, and the ascending and descending rates were slower. The AUC and TTP were increased, PI was decreased（P〈0.05）, and the MTY was increased（P〉0.05）. In SPECT group, GFR of hyperten- sion patients showed a downward trend compared with normal control group without statistically significant difference （P〉0.05）. Conclusion： Compared with SPECT, CEUS is more sensitive in assessment of early renal damage in patients with hyperten- sion, and can provide easy, safe and sensitive monitoring indexes for early renal damage.
