清肺化痰活血方治疗老年慢性阻塞性肺疾病的临床研究

摘　　要：目的:观察清肺化痰活血方治疗老年慢性阻塞性肺疾病痰瘀阻肺证的临床疗效。方法:选择148例老年慢性阻塞性肺疾病稳定期患者为研究对象,随机分为两组。对照组74例,采用异丙托溴铵气雾剂及沙丁胺醇气雾剂治疗;治疗组74例,在对照组基础上采用清肺化痰活血方治疗。比较治疗前后两组肺功能指标[一秒用力呼气容积(FEV1)、用力肺活量(FVC)]、动脉血气分析[二氧化碳分压(Pa CO2)、血氧分压(Pa O2)]、临床症状的变化情况,并评价两组临床疗效。结果:治疗组总有效率为90.5%(67/74),明显高于对照组75.7%(56/74),差异有统计学意义(P<0.05);与治疗前比较,治疗后两组FEV1/预计值、FVC、FEV1/FVC、Pa O2均明显上升,而中医证候积分均明显下降,且治疗组上述指标均优于对照组,差异有统计学意义(P<0.05)。结论:采用清肺化痰活血方治疗老年慢性阻塞性肺疾病痰瘀阻肺证,可显著提高临床疗效,缓解临床症状,促进患者肺功能的恢复。Objective： To observe the clinical efficacy of Qingfei Huatan Huoxue therapy （清肺化痰活血方, QFHTHXT） in treatment of syndrome of sputum stasis and obstructing lung in senile chronic obstructive pulmonary disease （COPD）. Methods： 148 selected elderly with COPD were randomly divided into two groups. Control group （n=74） took ipratropium bromide aerosol and salbutamol aerosol; research group （n=74） took QFHTHXT on the basis of control group. The lung function indexes [forced expiratory volume in one second （FEV1）, forced vital capacity （FVC）], arterial blood gas analysis [partial pressure of carbon dioxide （PaCOz）, partial pressure of oxygen （PaO2）], changes of clinical symptoms before and after treatment were compared and clinical efficacy of two groups were evaluated. Results： The total effective rate of research group was 90.5% （67/74）, significantly higher than 75.7% （56/74） of control group （P〈0.05）; Compared with pre-treatment, the FEV1/predicted value, FVC, FEV1/FVC, PaO2 of two groups significantly increased after treatment, while TCM symptom scores signifi- cantly decreased, and the above-mentioned indexes of research group were better than control group （P〈0.05）. Conclusion： QFHTHXT for syndrome of sputum stasis and obstructing lung in senile COPD can significantly increase clinical efficacy, relieve clinical symptoms, and promote recovery of lung function.
