摘 要:目的:探讨药师参与药物治疗管理中出院带药宣教的有效性和必要性。方法:分析目前医疗实践中出院带药的现状,阐述我院药师开展出院带药宣教的方法和流程,评估宣教工作的开展成效,总结相关成果和经验。结果与结论:目前医疗实践中出院带药具有流程简单、存在不合理用药隐患、医护人员缺乏专业化药品知识、患者安全用药意识较差等问题。我院出院带药宣教药师工作范围包括处方核对和用药宣教,宣教要点包括完善用药指导资料、药师走进临床等。在上述过程中,出院患者对我院出院带药宣教工作满意度高,且最希望药师多提供关于药品不良反应的宣教。相关成果为我院进一步优化药师工作模式指明了方向,此项工作加深了医患间的信任感,而宣教资料也成为了患者出院后的安全用药指南。出院带药宣教是药物治疗管理的重要组成部分,是促进出院患者合理用药的有效方法。OBJECTIVE: To explore the effectiveness and necessity of pharmacists providing medication instruction for discharged patients in therapy management. METHODS: The situation of discharged medication in medical practice was analyzed. The methods and processes of medication instruction for discharged patients in our hospital were elaborated, and the current situation and effectiveness of medication education were conducted. The related consequences and experiences were summarized. RESULTS & CONCLUSIONS: There were some problems, such as the process of drug taking out of hospital was simple and irrationalities of use of drug was found; medical staff lacked professional knowledge of drugs; patients had poor awareness of medication safety. The scope of medication instruction provided by pharmacists in our hospital included prescription checking and education, among which education involved the improvement of medication guidance materials, pharmacists participating in the clinical treatment, etc. In the above process, the discharged patients had high satisfaction with the propaganda and education work of our hospital, and hoped that pharmacists would provide more propaganda and education about ADR. Medication instruction for discharged patients indicated the direction for our hospital to further optimize and improve the pharmacists' working mode, and deepened the trust between doctors and patients. Publicity materials had become the safe medication principle for patients after discharge. Medication instruction for discharged patients plays a pivotal role in drug therapy management, which is an effective method to promote rational drug use in discharge patients.
