活血聪耳方联合丹参酮Ⅱ-A磺酸钠治疗分泌性中耳炎急性期的疗效及对中医证候积分的影响

摘　　要：目的研究活血聪耳方联合丹参酮Ⅱ-A磺酸钠(SⅡ-AS)治疗分泌性中耳炎(SOM)急性期的疗效及对中医证候积分的影响。方法选择从2015年9月~2016年7月在医院治疗的SOM急性期患者74例作为研究对象。依照简单数字随机表法将患者分成观察组37例以及对照组37例,两组均常规给予抗生素及抗过敏药物,同时给予复方呋喃西林滴鼻液和地塞米松注射液混合配液喷鼻,对照组在此基础上加用SⅡ-AS,观察组在对照组的基础上另给予活血聪耳方,两组疗程均为10天,不同时期的中医症候积分,听阈值的恢复率以及不良反应。结果观察组的总有效率是94.59%,明显高于对照组的78.38%,差异有统计学意义(χ~2=4.163,P=0.041)。治疗后观察组的耳痛、耳闷、耳鸣、听力下降、鼓膜内陷、中耳积液、纯音测听、舌象及脉象的中医症候积分均分别低于对照组差异均有统计学意义(P=0.000)。观察组听阈值的恢复率是91.89%,明显高于对照组的72.97%,差异有统计学意义(P=0.032)。治疗后观察组有2例患者出现皮疹,发生率是5.41%,对照组有1例患者出现皮疹,发生率是2.70%,两组相比,差异无统计学意义(P=0.556)。结论利用活血聪耳方与SⅡ-A S联合治疗SOM急性期患者,可取得较好的疗效,且能改善患者的中医证候及炎症状态,安全性较高,值得给予推广。Objective To study the effects of Huoxue conger decoction combined with Sodium II-A sulfonate on secretory otitis media at acute stage and TCM syndrome integral. Methods From September 2015 to June 2016 in the hospital treatment of 146 cases of patients with acute SOM as the research object. The patients were divided into observation group 73 cases and control group 73 cases according to the simple digital random table method, two groups were given conventional antibiotics and anti allergy drugs. At the same time given compound furacilin nasal drops and prednisone acetate injection mixed with liquid nasal spray, on this basis, the control group was added with sodium I1-A sulfonate,and the observation group based on the control group another given with Huoxue conger decoction. After 10 days of treatment, the efficacy of the two groups was compared, the scores of TCM syndromes, the levels of interleukin -2 （IL-2）, interleukin -6 （IL-6） and tumor necrosis factor alpha （TNF-ct）, and adverse reactions were compared between the groups. Results The total effective rate of the observation group was 94.59%, was significantly higher than the control group of 78.38%, the difference was statistically significant （x^2=4.163, P=0.041）. After treatment, the observation group of earache, aural fullness, tinnitus, hearing loss, tympanic membrane retraction, otitis media with effusion, pure tone audiometry, tongue and pulse of TCM syndrome integral were lower than those in the control group, the differences were statistically significant （P=0.000）. The recovery rate of the observation group was 91.89%, significantly higher than that of the control group 72.97%, and the difference was statistically significant （X~=4.573, P=0.032）. After treatment, the observation group had 2 cases of patients with rash, the incidence rate was 5.41%, and in the control group, there were 1 patients with rash, the incidence rate was 2.70%.
