不同鼻腔填塞材料对鼻内镜术后填塞效果的影响

摘　　要：目的比较凡士林纱布、医用聚乙烯醇、纳吸棉三种材料对鼻内镜术后填塞效果的影响。方法将2015年7月至2016年7月我科实施鼻内镜手术的45名患者作为研究对象,将患者随机分为三组,术后分别使用凡士林纱布、医用聚乙烯醇和纳吸棉进行填塞。对患者术后鼻腔胀痛程度、填塞操作时出血量、抽取鼻腔填塞物时难易程度、疼痛程度及出血量、心理状态分别进行评估和比较。结果针对各项观察项目,纳吸棉组均明显优于其他两组(P<0.05),医用聚乙烯醇组优于凡士林纱布组(P<0.05)。结论纳吸棉鼻腔填塞可明显降低患者鼻内镜术后的疼痛,减少出血量,减轻患者的焦虑状态,但价格最为昂贵。选择时必须考虑个体差异等综合因素的影响,做好知情同意。Objective To compare the clinical efficacy of vaseline gauze, medical polyvinyl alcohol and nasopore after endoscopic sinus surgery. Methods 45 patients were averagely derided into 3 groups randomly, vaseline gauze group, medical polyvinyl alcohol group and nasopore group. Pain after nasal packing, bleeding during packing process, difficulty, pain and bleeding during packing removal, and mental state in each group were evaluated and compared. Results For each index, the score in the nasopore group was significantly better than that in the vaseline gauze and the medical polyvinyl alcohol group （P〈0.05）, and the score in the polyvinyl alcohol group was also significantly better than that in the vaseline gauze group （P〈0.05）. Conclusion Nasopore could reduce pain after nasal package, decrease bleeding, and improve anxious state after endoscopic surgery, but it is much expensive. An informed consent is necessary after an overall consideration.
