磁共振扩散加权成像在鼻咽癌放疗疗效评价分析中的应用

摘　　要：目的研究磁共振扩散加权成像技术应用于鼻咽癌放疗疗效评价分析的可行性。方法在本院治疗的32例鼻咽癌患者,调强放射治疗前后均采用磁共振扩散加权成像(MRI-DWI)法进行病灶分析,比较放疗前后病灶区域表观扩散系数(Apparent Diffusion Coefficient,ADC)及病灶大小变化情况,评估MRI-DWI在鼻咽癌放疗疗效评估中的应用效果。结果 32例患者中,1例患者因严重放疗副作用而退出试验,其他患者均完成全程试验。MRI-DWI影像清晰显示,放疗后患者病灶显著变小,病灶区域的平均ADC显著增加,放疗前后相关指标变化差异具有统计学意义(P<0.05)。结论磁共振扩散加权成像技术可以有效地应用于鼻咽癌的放疗效果评估分析。Objective To investigate the feasibility of technology applied in the evaluation of therapeutic effect on nasopharyngeal carcinoma treated by radiotherapy via magnetic resonance image diffusion weight imaging （MRI-DWI） based on a clinical tria. Method Included in this study were 32 patients with nasopharyngeal carcinoma treated by intensity modulated radiation therapy in our hospital. Before and after radiotherapy, DWI-MRI was performed respectively for focus evahtation to analyze the range of apparent diffusion coefficient （ADC） and changes in the size of lesion in a comparative way in detail. Then, evaluated was the feasibility of MRI-DWI in the therapeutic effect evaluation on nasopharyngeal carcinoma treated by radiotherapy. Results Among these 32 cases, all completed the whole study course, with only one patient dropped out due to severe side effects following radiation. As shown from images of MRI-DWI, the size of lesion became smaller significantly among these cases following radiotherapy and ADC increased obviously in the region of lesion, with markedly statistical significance in the differences of change in the associated indicators before and after treatment （P〈0.05）. Conclusion DWI-MRI can be effectively utilized in the therapeutic effect evaluation on nasopbaryngeal carcinoma treated by radiotherapy.
