超声心动图对心脏血液充盈性囊肿的诊断价值

摘　　要：目的探讨心脏血液充盈性囊肿超声特点,为准确诊断及制定治疗策略提供参考。方法回顾分析我院经超声心动图检出并经病理证实的心脏血液充盈性囊肿5例,分析其临床症状、超声心动图特征、手术方式及预后。结果 5例临床症状均不典型,囊肿均为单发,其中4例囊肿与二尖瓣前叶相连,1例与三尖瓣前叶瓣环相连。超声心动图特征呈类圆形囊性"灯泡状",张力较高,囊壁为较薄且光滑的中高回声,其上可见高回声钙化,囊内为无回声,随着瓣膜或腱索的运动而摆动。5例患者中,3例仅进行单纯囊肿切除术,2例进行二尖瓣人工机械瓣膜置换术。结论心脏血液充盈性囊肿为成人罕见心脏良性肿瘤,根据超声心动图特征可对其做出诊断,有助于早期诊断及治疗策略制定。Objective To explore the characteristics of cardiac blood cyst to provide a reference for accurate diag- nosis and surgical procedure formulation. Methods The clinical symptoms, echocardiographic features, operation and prognosis were analyzed for 5 cases with cardiac blood cyst which detected by echocardiography and confirmed by pa-thology. Results All clinical symptoms were not typical and the cyst was isolated. 4 cases of cysts were attached to the anterior leaflet of the mitral valve, and the other case was connected to the tricuspid anterior annulus. Eehocardio- graphic feature demonstrated a single ＂balloon like＂ small cyst. The cyst had high tension with thin and smooth wall, calcification could be noted on cyst wall. The inside was none echogenicity area and the cyst moved and swung with the valve or chordae. All 5 cases were proved by surgery and pathology. The simple cystectomy was performed in 3 cases and the mitral valve artificial mechanical valve replacement was performed in 2 cases. Conclusions Cardiac blood cyst is rare cardiac benign tumor in adult, and can be diagnosed according to the characteristics of echocardiography. It con- tribute to early diagnosis and treatment strategies formation.
