超声心动图评价新生儿持续性肺动脉高压患者右室收缩功能

摘　　要：目的应用超声心动图定量新生儿持续性肺动脉高压患者(PPHN)的右室收缩功能。方法对经临床确诊的PPHN患儿34例,以及出生龄相匹配的新生儿30例,进行超声心动图右室收缩功能测量:(1)M型测量右室侧壁三尖瓣环收缩期峰值位移(TAPSE);(2)组织多普勒测量右室侧壁三尖瓣环的峰值速度(TAPSV);(3)经心尖四腔图测算右室面积变化分数(RVFAC)。结果 (1)PPHN组与对照组比较,年龄、胎龄、体质量、血压均无统计学意义;(2)PPHN组与对照组的左室收缩功能比较(LVEF、SV),差异无统计学意义(P>0.05);(3)TAPSE在PPHN组明显低于对照组:(6.19±0.80)mm vs(7.75±1.07)mm,差异有统计学意义,t=-6.63,P=0.000;(4)TAPSV在PPHN组明显低于对照组:(5.59±1.35)cm/s vs(6.64±1.36)cm/s,差异有统计学意义,t=-3.09,P=0.003;(5)RVFAC在PPHN组明显低于对照组:(46.52±5.87)%vs(51.60±4.66)%,差异有统计学意义,t=-3.79,P=0.000。结论新生儿持续性肺动脉高压者右室收缩功能减低,超声心动图可定量评估其变化,有助于临床治疗及随访。Objective To quantify the right ventricular systolic function in persistent pulmonary hypertension of the neonate（PPHN） by Echocardiography. Methods Retrospectively analyzed the clinical and echocardiographic data of 34 neonates with PPHN and 30 neonates without PPHN as control group. The tricuspid annular plane systolic ex- cursion（TAPSE）, the tricuspid annular plane systolic velocity （TAPSV） and right ventricular fractional area change （RVFAC） were compared between the two groups. TAPSE, TAPSV and RVFAC were all measured from a standard apical 4-chamber view. TAPSE and TAPSV were measured by M mode and Tissue Doppler, respectively. Results Compared with the control group, the PPHN group had no significant difference in age, gestational age, weight and blood pressure. There was no significant difference in left ventricular systolic function between PPHN and control group （P〉0.05）. Compared with control group, TAPSE, TAPSV and RVFAC in the PPHN group were significant- ly lower （P〈0.01）. Conclusions Right ventricular systolic function was decreased in PPHN group. Echocardio graphy can quantify the change and guide the management and follow-up of this group of patients.
