临床药师在老年痴呆科多学科团队诊疗工作中的实践与体会

摘　　要：目的:探讨临床药师在老年痴呆科多学科团队诊疗工作中的药学服务模式和方法。方法:通过典型案例,总结分析临床药师在老年痴呆科多学科诊疗工作中的药学实践。结果:临床药师通过审核医嘱、参与多学科查房、监测药品不良反应、开展患者用药教育、监测血药浓度及总结典型药历等多种形式开展全程化的药学服务,协助医师解决用药问题,促进合理用药,保障患者用药安全。结论:通过参与老年痴呆科的多学科诊疗工作,临床药师在促进老年患者合理用药中发挥了重要作用。Objective： To explore the modes and methods of pharmaceutical care of clinical pharmacists in the multidisciplinary team of senile dementia department. Methods： The work of clinical pharmacists in the multidisciplinary team of senile dementia department was summarized and analyzed by typical clinical cases. Results： The clinical pharmacists provided whole course pharmaceutical care including reviewing medical orders, taking part in ward rounds, analyzing adverse drug reactions, implementing medication education, monitoring drug concentration in blood, and summarizing typical medication cases. Through these activities, the clinical pharmacists assisted in the doctors to solve medication problems, promoted rational drug use, and ensured patient medication safety. Conclusion： Clinical pharmacists can develope an important role in promoting rational use of drugs in elderly patients through working in the multidisciplinary team of senile dementia department.
