无损伤处女膜宫腔镜手术治疗阴道斜隔综合征--附13例病例研究

摘　　要：目的探讨应用无损伤处女膜宫腔镜手术治疗青少年阴道斜隔综合征的有效性和安全性。方法收集2009—2016年期间就诊于中南大学湘雅三医院,并通过无损伤处女膜宫腔镜手术治疗的13例阴道斜隔综合征的青少年患者的病历资料,分析术中和术后随访情况,以了解该手术治疗技术的安全性和有效性。结果 13例患者均手术顺利,没有出现手术并发症,无处女膜损伤和阴道壁裂伤;术后痛经和(或)阴道流血等症状消失,术后3个月用宫腔镜行阴道内镜检查显示无阴道狭窄和斜隔创面粘连。结论无损伤处女膜宫腔镜手术治疗青少年阴道斜隔综合征是一种安全、微创和有效的治疗方式,在保留了处女膜完整性的同时也避免了尚未发育成熟的阴道壁的裂伤。Objective To evaluate the efficacy and safety of hysteroscopic management of OHVIRA （Obstructed hemivagina and ipsilateral renal agenesis）syndrome using a＂No-Touch＂technique in virgin adolescents.Methods The data from thirteen virgin adolescents with OHVIRA syndrome treated by＂No-Touch＂hysteroscopic surgery in the Third Xiangya Hospital of Central South University during 2009-2016, was collected and analyzed to evaluate efficacy and safety of the management.Results The surgery went smoothly in all patients. There were no surgical complications including injury of the hymen or vaginal wall. The symptoms of dysmenorrhea and vaginal bleeding were completely relieved postoperatively. There were no vaginal stenosis or adhesion between the cutting surface of the vaginal septum in all patients, which was revealed by the follow-up vaginoscopy using a hysteroscope at 3 months postoperatively. Conclusion Hysteroseopic management of OHVIRA syndrome using a＂No-Touch＂ technique in virgin adolescents is safe, microinvasive and effective, keeping the integrity of the hymen and avoiding the injury of the immature vaginal wall.
