甲状旁腺激素(1-34)对老年骨质疏松性肱骨近端骨折术后影响的临床研究

摘　　要：目的分析PTH在老年骨质疏松性肱骨近端骨折术后患者中的应用。方法选取老年骨质疏松性肱骨近端骨折患者100例,将100例患者随机分为治疗组(n=50)和对照组(n=50)。治疗组在锁定钢板内固定手术后加用PTH,对照组仅予锁定钢板内固定手术。对比分析治疗组与对照组术后各项指标差异,分析锁定钢板内固定手术联合PTH在骨质疏松性肱骨近端骨折老年患者中的应用。结果术后1周、2周时2组患者疼痛情况比较,治疗组NRS评分明显低于对照组(P<0.05),随访12个月时治疗组患者肩关节功能恢复情况明显优于对照组(P<0.05),治疗组术后并发症发生率明显低于对照组(P<0.05),治疗组骨折愈合时间、骨痂出现时间、住院时间及肿胀消退时间均短于对照组(P<0.05)。结论 PTH对老年骨质疏松性肱骨近端骨折患者术后安全有效,能够有效减轻患者疼痛,缩短病程,改善患者肩关节功能,降低术后并发症的发生率。Objective To analyze the application of PTH in osteoporotic proximal humeral fractures in elderly patients.Methods One hundred elderly patients with osteoporotic proximal humeral fractures were selected.They were randomly divided into treatment group（n = 50） and control group（n = 50）.Patients in the treatment group received PTH treatment after the locking plate internal fixation operation.Patients in the control group was treated with locking plate internal fixation only.The differences of indexes between the treatment group and the control group were compared and analyzed.The application of the associated locking plate internal fixation with PTH was analyzed in osteoporotic proximal humeral fractures in elderly patients.Results NRS score was significantly lower in the treatment group than that in the control group in 1 week and 2 weeks after surgery（P〈0.05）.At 12-month follow-up,the recovery of shoulder function in the treatment group was significantly better than that in the control group（P〈0.05）.The incidence of postoperative complications in the treatment group was significantly lower than that in the control group（P〈0.05）.The healing time,occurrence of callus,length of hospital stay,and the relieving time of swollen in the treatment group were shorter than that in the control group（P〈0.05）.Conclusion The application of PTH is safe and effective in elderly patients after osteoporotic proximal humeral fractures operation.It can effectively reduce the pain of patients,shorten the course of disease,improve the shoulder joint function,and reduce the incidence of postoperative complications.
