指背筋膜蒂皮瓣在手指皮肤软组织缺失修复中的应用价值

摘　　要：目的探讨指背筋膜蒂皮瓣在手指皮肤软组织缺失修复中的应用价值。方法将2016年3月~2017年3月110例手指皮肤软组织缺失修复患者作为对象,依据随机数字表法分组,各有55例。常规组采用传统的皮肤创面常规植皮手术进行治疗,研究组则采用指背筋膜蒂皮瓣进行修复治疗。比较两组手指皮肤软组织缺失修复总有效率;手指感觉评分、外形美观评分、关节活动评分;干预前后患者生活质量;感染、静脉危象、肿胀等发生率。结果研究组手指皮肤软组织缺失修复总有效率高于常规组(P<0.05);研究组手指感觉评分、外形美观评分、关节活动评分高于常规组(P<0.05);干预前两组生活质量相近(P>0.05);干预后研究组生活质量优于常规组(P<0.05)。研究组感染、静脉危象、肿胀等发生率低于常规组(P<0.05)。结论指背筋膜蒂皮瓣在手指皮肤软组织缺失修复中的应用价值高,可有效改善手指感觉和外观,促使关节活动恢复正常,减少感染、静脉危象、肿胀等的发生率,促进患者生活质量提高,值得推广。Objective To discuss the application value of dorsal fascial flap in finger skin and soft tissue defect in repair. Methods One hundred and ten patients with finger skin and soft tissue loss from March 2016 to March 2017 were selected and divided into groups according to the digital table method, with 55 cases in each group. The conventional group was given the traditional routine skin wound skin grafting treatment, the research group was given dorsal fasciocutaneous flap repair treatment. The total effective rate of finger skin and soft tissue defect repair in two groups was compared. Finger feeling score, appearance beauty score, joint activity score, the quality of life before and after intervention, infection rate, venous crisis and swelling rate were compared. Results The total effective rate of finger skin soft tissue defect repair in the study group was higher than that of the conventional group, P 〈 0.05; The finger feeling score, the appearance beauty score and＇the joint activity score in the study group were higher than those in the conventional group, P 〈 0.05. The quality of life in the two groups before the intervention was similar, P 〉 0.05; after intervention, the quality of life in the study group was better than that in the conventional group, P 〈 0.05; The incidence of infection, venous crisis and swelling in the study group was lower than that of the conventional group, P 〈 0.05. Conclusion The application of finger dorsal fasciocutaneous flap in the repair of soft tissue defects of the skin can effectively improve the sensation and appearance of the fingers, promote the normal return of joint activities, reduce the incidence of infection, venous crisis, swelling and so on, and promote the life of patients Improve the quality, it is worth promoting.
