机械通气联合盐酸氨溴索在治疗老年急性呼吸窘迫综合征中的临床效果观察

摘　　要：目的探讨老年急性呼吸窘迫综合征患者应用机械通气与盐酸氨溴索联合治疗的临床疗效。方法本研究选取2013年1月~2017年7月于我院接受分娩的72例老年急性呼吸窘迫综合征患者为试验对象,随机分为对照组与观察组各36例。对照组使用机械通气治疗,观察组使用机械通气与盐酸氨溴索联合治疗。对比两组治疗前后的动脉血气指标、血清细胞因子浓度及机械通气、ICU住院时间等。结果观察组治疗后的动脉血气指标均显著高于对照组患者,差异有统计学意义(P<0.05);观察组治疗后的TNF-α、IL-6浓度明显低于对照组患者,且IL-10明显高于对照组(P<0.05);观察组治疗后的机械通气、ICU住院时间均明显短于对照组患者(P<0.05)。结论机械通气与盐酸氨溴索联合治疗老年急性呼吸窘迫综合征患者的疗效较好,值得在临床治疗中应用。Objective To investigate the clinical effect of combined mechanical ventilation and ambroxol hydrochloride in elderly patients with acute respiratory distress Syndrome. Methods In this study,72 elderly patients with acute respiratory distress syndrome admitted to our hospital from January 2013 to July 2017 were randomly divided into control group and observation group,with 36 cases in each group.The control group was treated wigh mechanical ventilation,and the observation group was treated with ambroxol hydrochloride combined with mechanical ventilation.The arterial blood gas index, serum cytokine concentration and mechanical ventilation before and after treatment were compared between two groups. Results Arterial blood gas indexes in the observation group were significantly higher than those in the control group （P 〈 0.05）.After treatment,the concentrations of TNF-c~ and IL-6 in the observation group were significantly lower than those in the control group, and the levels of IL-lO in the observation group were significantly higher than those in the control group （P 〈 0.05）.The observation group after mechanical ventilation,ICU hospitalization was significantly shorter than the control group patients （P 〈 0.05）. Conclusion Mechanical ventilation and Ambroxol combined treatment of elderly patients with acute respiratory distress syndrome has a good effect and is worthy of clinical application.
