摘 要:目的评价利用手机为媒介进行血糖管理对2型糖尿病患者血糖及相关代谢指标的影响。方法全面收集国内外建库至2016年发表的相关文献,采用RevMan 5.3统计软件对各指标进行数据合并和敏感性分析。结果按照纳入标准,筛选出19篇随机对照试验研究(randomized controlled trials,RCT),共包括2726例2型糖尿病患者进行Meta分析。与对照组相比,采用手机管理的干预组糖尿病患者的空腹血糖、餐后2h血糖、糖化血红蛋白、体质量、收缩压、舒张压的差异均有统计学意义(均P<0.001)。然而,体质量指数[WMD=-0.06,95%CI(-0.17,0.05),P=0.31]、腰围[WMD=-0.53,95%CI(-1.09,0.02),P=0.06]、三酰甘油[WMD=-0.13,95%CI(-0.25,0.00),P=0.05]、总胆固醇[WMD=-0.33,95%CI(-0.73,0.06),P=0.10]、低密度脂蛋白[WMD=-0.14,95%CI(-0.31,0.02),P=0.09]、高密度脂蛋白[WMD=0.03,95%CI(-0.02,0.08),P=0.29]的差异均无统计学意义。结论利用手机对2型糖尿病患者进行血糖管理,在控制血糖、血压和体质量方面有益,对于血脂无明显影响,但仍需更多高质量大样本RCT加以验证。Objective To assess the effect of mobile phone intervention on blood glucose and metabolic indexes control in type 2 diabetes patients.Methods All relevant published articles in China and other countries' literature database were searched from the database establishment to 2016.RevMan 5.3 was used to merge data and analysis the sensitivity.Results There were 19 randomized control trials(RCT)selected based on including and excluding standards,which include the Meta-analysis of 2726 cases of type 2 diabetes patients.The statistical significance of fasting blood glucose,2 hpostprandial glucose,glycosylated haemoglobin,body mass,systolic blood pressure and diastolic blood pressure were found between control group and interventional group with mobile phone management(P<0.001).There was no statistical significance found in body mass index(WMD=-0.06,95%CI(-0.17,0.05),P=0.31),waist circumference(WMD=-0.53,95%CI(-1.09,0.02),P=0.06),triglyceride(WMD=-0.13,95%CI(-0.25,0.00),P=0.05),total cholesterol(WMD=-0.33,95%CI(-0.73,0.06),P=0.10),low density lipoprotein(WMD=-0.14,95%CI(-0.31,0.02),P=0.09)and high density lipoprotein(WMD=0.03,95%CI(-0.02,0.08),P=0.29).Conclusion Mobile phone intervention for patient with type 2 diabetes led to statistically significant improvement in glycaemic control,blood pressure and had benefit for weight loss,and has no obvious effect on blood lipids.However,more high-quality large sample RCT is needed to verify it.
