摘  要:目的探讨个性化营养干预对高龄孕妇妊娠期并发症、体质量控制及妊娠结局的影响。方法选取2015年1月-2016年12月100例于我院登记建卡的高龄孕妇,按随机数字表法分为对照组与观察组各50例,对照组予常规孕期指导,观察组在对照组基础上加用个性化营养干预,测定二组干预前后营养知识、饮食行为的变化,比较二组妊娠期并发症发生率,检测孕妇体质量控制情况,比较二组胎儿发育情况,记录宫高、胎儿双顶径,观察二组妊娠结局。结果(1)妊娠36周,二组营养知识、营养态度、饮食行为各维度评分均上升,观察组各维度评分高于对照组(P<0.05);(2)妊娠24周、妊娠36周,观察组体重均低于对照组,且其孕期体重增加低于对照组(P<0.05);(3)观察组子痫前期、妊娠期糖尿病发生率均低于对照组(0、2.00%vs.8.00%、14.00%)(P<0.05);(4)妊娠24周、妊娠36周,二组宫高、双顶径均增加,观察组妊娠36周宫高高于对照组(P<0.05);(5)观察组阴道分娩率高于对照组(70.00%vs.46.00%),剖宫产率、难产率低于对照组(30.00%、0vs.54.00%、8.00%),且其新生儿出生1min Apgar评分高于对照组,比较差异均有统计学意义(P<0.05)。结论个性化营养干预可降低高龄孕妇妊娠期并发症发生风险,控制其体重合理增长,改善产妇妊娠结局。Objective To investigate the effect of individualized nutritional intervention on pregnancy complications,body mass control and pregnancy outcomes in elderly pregnant women.Methods A total of 100 elderly pregnant women who were registered and established cards in author's hospital from May 2014 to August 2016 were randomly divided into the control group and the observation group with 50 cases in each group.The control group was given routine pregnancy guidance,and the observation group was additionally given individualized nutritional intervention on the basis of the intervention for control group.Changes in nutrition knowledge and dietary behaviors in the two groups before and after intervention were measured,and the incidence of pregnancy complications was compared between the two groups.Body mass control was determined,and fetal development was compared between the two groups.The fundal height and fetal biparietal diameter were recorded,and pregnancy outcomes of the two groups were observed.Results After 36 weeks of pregnancy,scores of nutrition knowledge,nutritional attitude and dietary behaviors of two groups were increased,and the scores of different dimensions in observation group were higher than those in control group(P〈0.05).The weight of the observation group was lower than that of the control group after 24 weeks and 36 weeks of pregnancy,and the weight gain during pregnancy was lower than that of the control group(P〈0.05).The incidence rates of preeclampsia and gestational diabetes in the observation group were lower than those in the control group(0,2.00% vs 8.00%,14.00%)(P〈0.05).The fundal height and fetal biparietal diameters of two groups were increased after 24 weeks and 36 weeks of pregnancy,and the fundal height of observation group after 36 weeks of pregnancy was higher than that of control group(P〈0.05).In observation group,the vaginal delivery rate was higher than that in the control group(70.00% vs 46.00%),and the cesarean section rate and dystocia rate we
