摘 要:目的研究视网膜电图振荡电位(OPs)总振幅及尿微量白蛋白(U-mAlb)水平在糖尿病视网膜病变(DR)各期中的变化情况,探讨将这些指标应用于临床筛查的可能性。方法测定健康对照组、临床前期组及轻、中、重度非增生性DR组和增生性DR组等6组的闪光视网膜电图(FERG)的OPs总振幅及U-mAlb排出量,并行统计学比较。结果 OPs总振幅及U-mAlb排出量与DR各组间有显著相关性。OPs总振幅随着DR程度的加重而减小,6组间差别均有统计学意义(P<0.01)。U-mAlb排出量在健康对照组、临床前期组及轻度非增生性DR组中差别无统计学意义,但在中度非增生性DR组、重度非增生性DR组及增生性DR组中差别有统计学意义(P<0.01),且病变越严重,U-mAlb排出量上升越快。结论 OPs总振幅可作为DR的定量指标,对诊断有重要价值,尤其是早期DR,敏感性较高;U-mAlb排出量与DR的严重程度相关。Objective To investigate the total oscillatory potentials(OPs) amplitude of the elec- troretinogram and Urinarymicroalbumin (U-mAIb) levels in diabetic retinopathy (DR) changes in different phases; to explore the possibility of using these indicators in clinical screening. Methods The total OPs amplitude of flash electroretinogram(FERG) and U-mAlb output were measured in the following groups. healthy control groups, subclinical DR groups, mild, medium, and severe non-proliferative DR groups and proliferative DR groups. Then the results were statistically analyzed. Results There was a significant correlation between the total amplitude of OPs and the amount of U-mAlb excreted in different DR groups. The total amplitude of OPs decreased with the aggravation of DR, and the differences between the six groups were statistically significant. The differences of U-mAlb excretion among healthy control groups, subclinical DR groups and mild non-proliferative DR groups were not statistically significant, but there were statistical significance among moderate non-proliferative DR groups, severe non-proliferative and pro- liferative DR group, while the U-mAlb emissions increased faster when the lesions were more severe. Conclusions The total amplitude of OPs can be used as a quantitative index for DR with high sensitivity, which is of great value for the diagnosis of DR, especially for early DR. The amount of U-mAlb excreted was related to the severity of DR. OPs is proposed for early DR screening and U-mAlb can be used to de- termine the severity of the disease and prognosis.
