摘  要:目的观察脓毒症肺损伤大鼠肺组织白细胞介素17F(Interleukin-17F,IL-17F)、肺组织高迁移率族蛋白B1(High mobility group protein box-1,HMGB1)、血清降钙素原(Procalcitonin,PCT)的水平,探讨雾化吸入灭活草分枝杆菌治疗脓毒症肺损伤机制。方法 48只清洁级健康雄性Wistar大鼠随机分成假手术组(Sham组)、脓毒症肺损伤组(CLP组)、草分枝杆菌F.U.36注射液雾化吸入治疗组(F.U.36组),每组16只。于手术后24、48h分别检测IL-17F、PCT、HMGB1浓度,光镜观察肺组织病理变化。结果与Sham组比较,CLP组及F.U.36组在24、48h IL-17F、PCT、HMGB1水平均显著升高,差异有统计学意义(P<0.01);与CLP组同期比较,F.U.36组IL-17F、PCT、HMGB1浓度均明显下降,差异亦有统计学意义(P<0.01)。光镜下CLP组和F.U.36组可见肺损伤,CLP组较F.U.36组损伤明显。结论雾化吸入灭活草分枝杆菌可降低脓毒症肺损伤时相关炎症因子水平,对肺有保护作用。OBJECTIVE To observe the levels of interleukin-17F (IL-17F) ,high mobility group protein box-1 (HMGB1) ,procalcitonin(PCT) release in septic rats with acute lung injury and to explore the mechanism of inhaled inactivated-Mycobacterium Phlei in the treatment of septic acute lung injury. METHODS A total of 48 male Wistar rats of clean grade were randomly divided into the sham-operation group (Sham) , the septic acute lung injury model group (CLP), and the inhaled inactivated-Mycobacterium Phlei F. U. 36 treatment group (F. U. 36 ), 16 in each group. Concentrations of IL-17F, HMGB1 and PCT expression were detected in each group at 24h and 48h after oper- ation. Pathological changes of the lung were observed under light microscope. RESULTS Concentrations of IL-17F, HMGB1 and PCT at 24h and 48h after operation were significantly higher in the CLP group and in the F. U. 36 group than in the Sham group (P 〈0. 01 ). Concentrations of IL-17F,HMGB1 and PCT at 24h and 48h after operation were significantly lower in the F. U. 36 group than in the CLP group (P 〈0. 01 ). Lung injury occurred in the CLP and the F. U. 36 group under light microscope, And this injury was more severe in the CLP group than in the F. U. 36 group. CONCLUSION inhaled inactivated-Mycobacterium Phlei could reduce the level of sepsis-related inflammatory cytokines and protect lung in septic rats.
