分级诊疗推进过程中双向转诊问题及对策研究——以某市为例

摘　　要：目的通过了解某市上级医院与社区卫生服务机构的基本服务情况,分析双向转诊中存在的问题,为某市双向转诊制度的进一步完善提出相关建议。方法采用文献法和问卷调查方法收集资料,通过重点调查与抽样调查相结合,对某市3家综合性医院、7家社区卫生服务机构进行问卷调查。结果社区卫生服务机构与上级医院的医务人员在学历和职业水平上存在差距;社区卫生服务机构的日常接诊量低于上级医院的接诊量;患者重复检查、医疗资源不足、患者转诊意愿、医疗风险、医院经济效益成为影响双向转诊有效落实的主要因素。结论通过加强各级医疗机构诊疗信息互认,提高双向转诊效率,政策制度跟进等措施解决双向转诊问题。Objective Put forward relevant suggestions to perfect the two-way referral system of a city through investigating basic services of the city’s superior hospital and community health service and analyzing the existing problems in the two-way referral system.Methods Adopt literature review and questionnaire survey method.The questionnaire of three comprehensive hospitals and seven community health service organizations were carried out by way of key investigation and sampling survey.Results There were distance in the level of education and occupation of medical staff in community health services and hospital,daily admissions amount of community health service institutions was less than the superior hospital.Factors like patients with repeated inspection,lack of medical resources,referral willingness of patients,medical risks,and hospital economic benefits greatly influence the effective implementation on two-way referral.Conclusion Take measures like strengthening mutual recognition of medical information among medical institutions at all levels,improving the efficiency of two-way referral,and following up relevant policies and systems to solve the problem in the two-way referral system.
