痛风膏外敷治疗痛风性关节炎湿热蕴结证的疗效观察

摘　　要：目的观察痛风膏外敷治疗急性痛风性关节炎的临床疗效和安全性,为临床应用中医特色治疗该类疾病提供依据。方法将急性痛风性关节炎湿热蕴结证患者128例随机分成2组,每组64例,对照组采用低嘌呤饮食、碱化尿液及非甾体抗炎类等西医药物治疗;试验组除采用上述常规治疗外,加用本院自制中药痛风膏进行外敷治疗。治疗一周后,比较两组治疗前后关节肿痛及功能评分(包括VAS疼痛评分、关节肿胀指数及关节活动评分)、临床疗效及不良反应。结果试验组总有效率为95.3%,对照组总有效率为81.3%,两组比较差异有统计学意义(P<0.01)。两组在改善VAS视觉疼痛评分、关节肿胀指数、关节活动评分方面,每组内治疗前后比较差异有显著统计学意义(P<0.01);治疗后两组间比较,试验组优于对照组,差异有显著统计学意义(P<0.01)。两组治疗期间均未见严重不良反应发生,试验组的不良反应发生率低于对照组(P<0.05)。结论外敷痛风膏结合西药治疗急性痛风性关节炎湿热蕴结证,能更有效改善临床症状,提高临床疗效,且安全性高,值得临床推广应用。Objective To observe the clinical efficacy and safety on treating acute gouty arthritis using traditional Chinese medicine Gout Ointment,which provides the basis for the clinical application of traditional Chinese medicine. Methods 128 cases of acute gouty arthritis with damp-heat amassment syndrome were randomly divided into two groups: the control group and the experimental group. There were 64 persons in each group. All Patients received routine Western medicine treatment of using low purine diet,alkaline urine and non-steroidal anti-inflammatory,but the experimental group received self-made traditional Chinese medicine Gout Ointment for external application. After a week's treatment,the joint swelling and pain,function score( Including VAS visual analogue pain scale,joint swelling index and joint function score),therapeutic evaluation and adverse reaction were compared between the two groups pre and post treatment. Results 1. The total effective rate is 95. 3% in the experimenta group and 81. 3% in the control group,a P value of less than 0. 01( P <0. 01) is considered statistically significant. 2. There is significant difference in improving VAS visual pain score,joint swelling index and joint function score was statistically significant( P < 0. 01) in two group,and the experimental group shows better result than control group after a week's treatment. 3. There was no serious adverse reaction between the two groups. The incidence of adverse reactions in experimental group is lower than of control group( P < 0. 05). Conclusion The traditional Chinese medicine Gout Ointment combined with western medicine treatment of acute gouty arthritis with damp-heat amassment syndrome has a better effect on relieving clinical symptoms and improving clinical efficacy with a high safety,which is worthy of clinical application.
