中医辨证施治联合桥接组合式内固定系统治疗股骨干骨折的临床研究

摘　　要：目的:分析和观察中药结合桥接组合式内固定系统治疗股骨干骨折的临床效果。方法:选取2013年1月-2017年1月在我院治疗的股骨干骨折患者80例,随机将其分为治疗组(40例)和对照组(40例),对照组患者采用锁定钢板内固定,术后按骨折三期辨证用药分别内服正骨1、2、3号片(本院制剂室生产)的方法进行治疗,治疗组患者采用桥接组合式内固定系统内固定,术后在对照组用药基础上三期分别加用不同的中药汤剂进行治疗,观察两组内固定是否松动、断裂,骨折愈合情况,骨折愈合时间等。结果:两组骨折愈合时间比较,P<0.05,治疗组临床愈合时间优于对照组,术后并发症少,临床有效率优于对照组,两组比较,差异有统计学意义。患肢功能恢复情况根据Johner-Wruh标准评价:治疗组优35例,良2例,中3例,无差病例,优良率达92.5%;对照组优28例,良5例,差7例,优良率达82.5%。结论:桥接组合式内固定系统作为一种新的有限接触内固定器材,整体锁定、多维固定,有效控制旋转和螺钉拔出,对固定段无应力遮挡效应,有效保护骨折端血供,术后中医新的三期辨证用药治疗,能防止切口感染,促进骨痂生长,消除骨折并发症,有利于骨折的愈合。Objective: To analyze and observe the clinical effect of traditional Chinese medicine with combined bridge combined internal fixation system in treating femoral shaft fracture. Method: 80 cases of femoral fracture treated in our hospital from January, 2013 to January, 2017 were selected and randomly divided into the treatment group(40 cases) and the control group(40 cases). The control group was treated by locking plate fixation and oral administration of dialectical medication of 1, 2, 3 tablet(the hospital prescribed) after operation. The treatment group was treated by bridge combined internal fixation system and the TCM decoction more. The internal fixation loosening, fracture, fracture healing, fracture healing time in two groups were observed. Results: The fracture healing time in the two groups was compared, P < 0.05. The clinical healing time in the treatment group was better than the control group, the postoperative complications were fewer, the clinical efficiency was better than the control group, and the differences between the two groups were statistically significant. The functional recovery of the affected limb was evaluated according to Johner Wruh criteria: in the treatment group 35 cases was excellent, good in 2 cases, moderate in 3 cases, and there was no bad case, the excellent and good rate was 92.5%; in the control group 28 cases was excellent, good in 5 cases, poor in 7 cases, and the excellent and good rate was 82.5%. Conclusion: The bridge combined fixation system as a kind of limited contact with new materials for internal fixation had the advantages of whole locking, multidimensional fixed, effective control of rotation and screw pull-out, no stress shielding effect on the fixed period, and it could effectively protect the blood supply of fracture, postoperative TCM three dialectical medication could prevent from wound infection and promote the growth of callus, eliminate the complication of fracture, which helped the healing of fracture.
