妊娠合并糖尿病孕产妇不同时期加用胰岛素治疗对妊娠结局的影响

摘　　要：目的探讨妊娠合并糖尿病孕产妇不同时期加用胰岛素治疗对妊娠结局的影响。方法选取2016年1—12月该院收治的220例妊娠合并糖尿病的孕产妇,随机将其分为两组,对照组患者在孕周>32周时加用胰岛素治疗,研究组患者在孕周<32周时加用胰岛素治疗,并比较不同孕产妇血糖指标和妊娠结局。结果与治疗前比较,研究组和对照组孕产妇空腹血糖和餐后2 h血糖均明显降低(P<0.05),其中研究组孕产妇降低更为明显(P<0.05);研究组孕产妇剖宫产率、羊水过多、产后感染、产后出血以及妊娠期高血压等并发症发生率明显低于对照组(P<0.05);研究组新生儿低血糖、巨大儿、新生儿窒息等发生率明显低于对照组(P<0.05)。结论在孕产妇孕周32周前使用胰岛素不仅有助于改善妊娠合并糖尿病孕产妇血糖水平,同时还能明显减少妊娠期并发症和新生儿不良结局发生率。Objective To study the effect of insulin treatment of pregnant women with diabetes at different periods on the gestational outcome. Methods 220 cases of pregnant women with diabetes admitted and treated in our hospital from January to December 2016 were selected and randomly divided into two groups, the control group added the insulin for treatment when the pregnant week 32 weeks, while the research group added the insulin for treatment when the pregnant week 〉32, and the blood glucose indicator and gestational outcome of the two groups were compared. Results The fasting blood glucose and postprandial 2 h blood glucose in the research group and in the control group obviously decreased compared with those before treatment（P〈0.05）, more obviously in the research group（P〈0.05）, the incidence rate of complications such as cesarean section rate, excessive amniotic fluid, postpartum infection, postpartum hemorrhage and pregnancy induced hypertension in the research group was obviously lower than those in the control group（P〈0.05）, and the incidence rate of neonatal hypoglycemia, macrosomia and neonatal asphyxia in the research group was obviously lower than that in the control group（P〈0.05）. Conclusion The insulin treatment for pregnant women whose pregnant week 32 weeks contributes to improving the blood glucose level and obviously reduce the incidence rates of gestational complications and adverse outcomes of newborns.
