彩色多普勒对早期糖尿病肾病的诊断研究

摘　　要：目的分析彩色多普勒对早期糖尿病肾病的诊断价值。方法选取该院2016年1—12月收治的100例糖尿病患者,50例患者为纯糖尿病组(24 h尿蛋白正常),50例患者为糖尿病肾病组(24 h尿蛋白正常>30 mg),同时选取50名健康者作对比试验为对照组。对两组患者和健康对照组进行彩色多普勒超声检查,并对患者的肾脏长度、宽度、厚度进行记录,从而计算患者肾脏体积(计算公式:长径×宽度×厚度×π/6),同时记录患者肾动脉收缩期的最高流速(Vsmax)、舒张期的最低流速(Vdmin)和血流阻力指数,对两组患者和健康对照组结果进行对比。结果对两组患者和健康对照组的肾脏长度、宽度、厚度和体积进行对比,纯糖尿病组和糖尿病肾病组的肾脏体积差异有统计学意义(P<0.05)。对两组患者和健康对照组的Vsmax、Vdmin、RI进行对比,糖尿病肾病组与其他两组的Vsmax、Vdmin差异有统计学意义(P<0.05)。结论临床应用彩色多普勒超声对早期糖尿病患者的肾脏进行检查,有助于及时有效地发现早期糖尿病肾病,从而对后期的针对性治疗提供有效的临床指证,加之彩色多普勒超声临床普及度高,因此这种检测手段值得临床广为推广。Objective To analyze the value of color Doppler in diagnosis of early diabetic nephropathy. Methods 100 cases of diabetes patients admitted and treated in our hospital from January to December 2016 were selected and divided into two groups with 50 cases in each, including the simple diabetes group（ normal 24 h urine protein） and diabetic nephropathy group（normal 24 h urine protein 30 mg）, at the same time, 50 cases of healthy patients were selected as the control group, and both groups were examined by the color Doppler, and the renal length, width and thickness of patients were recorded, thus calculating the renal volume（calculation equation： major diameter×width×thickness×π/6）, at the same time,the Vsmax, Vsmax and blood flow resistance index of patients were recorded, and the results of the two groups were compared. Results The differences in the renal length, width, thickness and volume between the two groups were obvious（P0.05）,and there were differences in the Vsmax and Vdmin between the diabetic nephropathy group and the other two groups（P0.05）.Conclusion The clinical application of color Doppler ultrasound in examining the renal of diabetics patients contributes to discovering the early diabetic nephropathy in time and effectively, thus providing effective clinical signs of targeted treatment in the advanced period, besides, the clinical popularity of color Doppler ultrasound is high, it is worth clinical promotion.
