血管内超声指导慢性完全闭塞冠状动脉病变介入治疗的疗效探讨

摘　　要：目的分析探讨血管内超声指导慢性完全闭塞性冠状动脉病变介入治疗的临床治疗效果。方法方便选取该院在2015年1月—2017年1月所收治的120例行冠状动脉造影检查诊断为慢性完全闭塞病变(CTO)并行经皮冠脉介入治疗(PCI)的住院患者作为研究对象,将其随机分为研究组和对照组,各60例;研究组患者施以血管内超声(IVUS)指导PCI治疗,对照组患者未使用血管内超声指导,然后对比两组患者临床治疗效果。结果研究组患者经血管内超声(IVUS)指导PCI治疗,其手术成功率为95.00%(57/60),比对照组患者的73.33%(44/60)明显偏高,组间差异有统计学意义(P<0.05);对所有患者在治疗后经6个月随访,结果表明研究组患者的并发症发生率为5.00%(3/60),比对照组患者的41.67%(25/60)明显偏低,组间差异有统计学意义(P<0.05)。结论血管内超声指导慢性完全闭塞性冠状动脉病变介入治疗的临床效果显著,相应手术成功率高,并发症发生率低,安全可靠,因而具有广阔的临床应用价值。Objective To analyze the curative effect of intravascular ultrasound in guiding the intervention therapy of chronic total occlusion coronary arterial lesions. Methods 120 cases of chronic total occlusion inpatients with PCI admitted and treated in our hospital from January 2015 to January 2017 were selected as the research objects and randomly divided into two groups with 60 cases in each, the PCI treatment guided by IVUS was conducted in the research group, while intravascular ultrasound guidance was not applied in the control group, and then the clinical treatment effect was compared between the two groups. Results The operation success rate in the research group was obviously higher than that in the control group after treatment, [95.00%(57/60) vs 73.33%(44/60)], and the difference was obvious(P<0.05), and the 6-month follow-up showed that the incidence rate of complications in the research group was obviously lower than that in the control group, [5.00%(3/60) vs 41.67%(25/60)], and the difference was statistically significant(P <0.05). Conclusion The clinical effect of intravascular ultrasound in guiding the intervention therapy of chronic total occlusion coronary arterial lesions is obvious, and the corresponding operation success rate is high with low incidence rate of complications and it is safe and reliable, and it is of broad clinical application value.
