美皮康泡沫银联合3M透明薄膜在深静脉置管护理中的应用

摘　　要：目的探讨美皮康泡沫银联合3M透明薄膜在深静脉置管护理中的应用效果。方法方便选取2016年6月—2017年3月遵义医学院附属医院烧伤整形外科64例深静脉置管的患者,随机分为观察组和对照组各32例,观察组患者使用美皮康泡沫银联合3M薄膜覆盖深静脉穿刺点,对照组只采用3M透明薄膜覆盖深静脉穿刺点,然后两组均用"高抬平举法"固定深静脉置管外露导管。观察并比较两组敷贴松脱,更换敷料时穿刺点局部不良反应(红、肿、渗血及渗液)及周围皮肤是否疼痛破损及出血,患者的局部舒适情况。结果观察组导管滑脱率3.1%,对照组导管滑脱率15.6%,差异无统计学意义(P>0.05)。观察组敷料松脱率6.3%、穿刺点不良反应发生率6.3%、穿刺点周围皮肤不良情况0.0%、穿刺点细菌培养6.3%、人均经济消费(9.45±0.17)元/d,均低于对照组的46.9%、46.9%、18.8%、62.5%、(10.34±0.22)元/d;观察组主观舒适度90.9%,高于对照组84.3%,差异有统计学意义(P<0.05)。结论美皮康泡沫银联合3M透明薄膜在深静脉置管护理中能有效的固定深静脉置管、预防穿刺点的不良反应及周围皮肤的完整性、促进患者的舒适。Objective This paper tries to investigate the application of mepilex foam silver combined with 3 M transparent film in deep venous catheter in nursing. Methods 64 cases of deep venous catheter treated in this hospital from June 2016 to March 2017 were convenient selected and randomly divided into the observation group and the control group, with 32 cases in each group, the observation group was given mepilex foam silver combined 3 M film covering deep vein puncture point, the control group using only 3 M transparent film covering deep vein puncture point, and both groups adopted ＂high flat method＂ of deep venous catheter fixed exposure. The local comfort of the puncture points, loosening and changing dressings were observed and compared. The local adverse reactions（redness, swelling, oozing of blood and exudate） and the surrounding skin pain, breakage and bleeding were compared between the two groups. Results The catheter slippage rate of the observation group was 3.1%, and the catheter slip rate of the control group was 15.6%, without significant difference（P〈0.05）.The loose dressing rate in the observation group was 6.3%, the puncture point was 6.3%, the incidence of adverse reactions and the puncture point surrounding skin was 0.0%, the puncture point bacteria culture was 6.3%, per capita consumption was（9.45±0.17）yuan/d, lower than the control group of 46.9%,46.9%,18.8%,62.5%,（10.34±0.22）yuan/d; the subjective comfort of observation group was 90.9%, higher than 84.3% in the control group and with significant difference（P 0.05）.Conclusion Mepilex foam silver combined with 3 M transparent film in deep vein catheter nursing can effectively fix deep venous catheter, prevent adverse reactions and peripheral skin integrity, and promote the patient＇s comfort.
