不同手术方式治疗卵巢良性囊肿的临床效果对比分析

摘　　要：目的对不同手术治疗卵巢囊肿的临床疗效进行观察,通过对比,对悬吊式腹腔镜手术治疗卵巢囊肿的临床应用价值进行评价。方法方便选取在2015年12月—2016年12月期间来该院进行手术治疗的卵巢囊肿患者60例的临床资料进行回顾,根据不同的手术方式,将其分为3组,既悬吊组、气腹组和开腹组,每组患者各20例,对3组患者术中出血量、术后疼痛等指标进对比分析。结果和气腹组悬相比较,悬吊组住院时间(70.0±6.43)min、较少的出血量(105.0±6.70)m L,排气时间为(15.0±1.60)h,两组患者住院时间和术后肛门排气时间差异无统计学意义(P>0.05)。术后12 h后,开腹组腹部切口疼痛评分平均(6.2±1.7)分,明显高于其他两组,差异有统计学意义(P<0.05);气腹组非切口疼痛发生率达42.5%,明显高于其他两组,差异有统计学意义(P<0.05)。结论悬吊式腹腔镜卵巢囊肿手术的优势是具有较快的康复速度,使腹腔镜手术的可操作性提升,可在基层医院推广和普及微创技术。Objective This paper tries to analyze the clinical efficacy of different surgical treatment for ovarian cysts.Through contrast,the clinical value of gasless laparoscopic surgery in the treatment of ovarian cysts was evaluated.Methods The clinical data of 60 cases of ovarian cyst patients from December 2015 to December 2016 in this hospital for surgical treatment were convenient reviewed,according to the different operation mothods,they were divided into 3 groups,namely the suspension group,pneumoperitoneum group and laparotomy group,with 20 cases in each group,the amount of hemorrhage,postoperative pain and other index of the three groups of patients were compared and analyzed.Results Compared with pneumoperitoneum group,the hospitalization time was（70.0 ±6.43）min,blood loss was（105.0±6.7）mL,exhaust time was（15.0±1.6）h in the suspension group,and there were no differences in hospitalization time and postoperative exhaust time in two groups of patients,with no statistically significant difference（P 〉0.05）.12 h after operation,the average pain score of abdominal incision in the laparotomy group was（6.2±1.7）points,significantly higher than the other two groups,the difference was statistically significant（P 〈0.05）; pain incidence rate of non incision in the pneumoperitoneum group was42.5%,significantly higher than the other two groups,the difference was statistically significant（P〈0.05）.Conclusion The gasless laparoscopic ovarian cyst surgery has the advantage of fast recovery,which can improve the operability of laparoscopic surgery,and the minimally invasive techniques can be popularized and promoted in primary hospital.
