前列地尔保护体外循环肺损伤30例临床研究

摘　　要：目的观察前列地尔对体外循环肺损伤的保护作用。方法选取60例行体外循环心内直视瓣膜置换手术患者,随机分为治疗组和对照组,各30例。两组患者予相同麻醉方法及手术治疗,治疗组患者术中给予前列地尔注射液。结果治疗后,两组患者氧合指数(OI)、阻力指数(RI)均明显升高,且治疗组升高幅度大于对照组(P<0.05);治疗后,两组患者丙二醛(MDA)水平均明显降低,超氧化物歧化酶(SOD)水平均明显升高,且治疗组变化幅度均高于对照组(P<0.05);治疗后,两组患者血清白细胞介素6(IL-6)和白细胞介素8(IL-8)表达均明显上升(P<0.05),且治疗组上升幅度小于对照组(P<0.05)。结论前列地尔可通过降低体外循环瓣膜置换血浆炎性因子水平,提高氧合指数,降低呼吸指数,清除氧自由基,在多个环节对体外循环所致肺损伤提供保护作用,临床疗效好、安全性高。Objective To observe the effect of alprostadil for protecting the lung injury during cardiopulmonary bypass（ CPB）.Methods Totally 60 patients undergoing valve replacement operation with CPB were selected and randomly divided into the observation group and the control group,30 cases in each group. The patients in the two groups were given the same anesthesia method and surgical treatment,and the treatment group was given Alprostadil Injection during the operation. Results After treatment,the oxygenation index（ OI）and resistance index（ RI） were increased in the two groups,and the increase in the treatment group was higher than that of the control group（ P 0. 05）. After treatment,the levels of malondialdehyde（ MDA） and superoxide dismutase（ SOD） in the two groups were increased significantly,and the change in the treatment group was higher than that in the control group（ P 0. 05）. After treatment,the levels of serum interleukin-6（ IL-6） and interleukin-8（ IL-8） in the two groups were increased significantly,and the increase in the treatment group was lower than that in the control group（ P 0. 05）. Conclusion Alprostadil can reduce the level of inflammatory factors in patients undergoing valve replacement operation with CPB,improve oxygenation index,reduce respiratory index,clearing oxygen free radicals,provide a protective effect on lung injury induced by cardiopulmonary bypass in many processes,it has good clinical effect and high safety,which is worthy of clinical promotion.
