益气化瘀补肾方治疗绝经后妇女原发性骨质疏松症及对骨代谢标志物的影响

摘　　要：目的探讨益气化瘀补肾方治疗绝经后妇女原发性骨质疏松症的临床疗效及对骨代谢标志物的影响。方法选择医院2015年1月至2017年6月收治的原发性骨质疏松症绝经后妇女80例,随机分为对照组和观察组,各40例。对照组患者采用常规西药治疗,观察组患者在此基础上加用益气化瘀补肾方。两组疗程均为3个月。比较两组患者全身骨密度(BMD)、中医证候积分、血清骨转换标志物[碱性磷酸酶(ALP)、Ⅰ型原胶原N-端前肽(PINP)和Ⅰ型胶原交联C-末端肽(S-CTX)]、血钙、血磷、血肌酐、甲状旁腺激素(PTH)和维生素D_3水平。结果治疗后,与对照组相比,观察组BMD值、中医证候积分均明显增加,血清骨转换标志物水平明显降低(P<0.05);两组患者治疗前后血钙、血磷、血肌酐、PTH和维生素D_3水平比较均无显著性差异(P>0.05)。结论益气化瘀补肾方治疗绝经后妇女原发性骨质疏松症疗效显著,可提高BMD值,改善中医证候积分,降低骨代谢标志物,安全性好,值得临床推广。Objective To investigate the clinical effect of Yiqi Huayu Bushen Recipe in the treatment of postmenopausal women patients with primary osteoporosis and its effect on markers of bone metabolism. Methods Totally 80 postmenopausal women patients with primary osteoporosis from January 2015 to June 2017 were selected and randomly divided into the control group and the observation group,40 cases in each group. The control group was treated with conventional Western medicine,on this basis,the observation group was treated with Yiqi Huayu Bushen Recipe. The course was 3 months. The bone mineral density（ BMD）, TCM symptom score, the levels of serum bone tumor markers [ alkaline phosphatase（ ALP）,procollagen type Ⅰ N-propeptide（ PINP） and C-terminal telopeptide of type Ⅰ collagen cross-linking（ S-CTX） ], serum calcium, phosphorus, creatinine, parathyroid hormone（ PTH） and vitamin D3（ Vit D3） were recorded and compared. Results After treatment,compared with the control group,the value of BMD and score of TCM syndrome in the observation group increased obviously, and the level of serum bone turnover markers decreased significantly（ P〈0. 05）. There was no significant difference in the levels of serum calcium,phosphorus,creatinine,PTH and Vit D_3 before and after treatment in the two groups（ P〉0. 05）.Conclusion Yiqi Huayu Bushen Recipe in the treatment of postmenopausal women patients with primary osteoporosis has good effect and safety, it can improve the value of BMD and score of TCM syndrome,reduce the markers of bone metabolism, which is worthy of clinical promotion.
