二甲双胍联合孕激素治疗子宫内膜不典型性增生合并胰岛素抵抗的疗效

摘　　要：目的探讨二甲双胍联合孕激素治疗子宫内膜不典型性增生合并胰岛素抵抗的临床疗效。方法将100例子宫内膜不典型性增生合并胰岛素抵抗患者按随机数字表法分为对照组与观察组,每组50例,对照组给予孕激素(醋酸甲地孕酮分散片)治疗,观察组给予二甲双胍联合孕激素治疗。治疗3、6个月后比较2组周期排卵率、单卵泡发育率、妊娠率及糖脂代谢、胰岛素、性激素水平变化情况。结果治疗后观察组周期排卵率、单卵泡发育率及妊娠率均较对照组显著升高(P<0.05);治疗3、6个月后观察组糖脂代谢、胰岛素、性激素水平均较对照组显著改善(P<0.05)。结论二甲双胍联合孕激素治疗子宫内膜不典型性增生合并胰岛素抵抗具有良好的临床疗效,可提升妊娠率,改善胰岛素水平。Objective To investigate the effects of metformin combined with progesterone on endometrial atypical hyperplasia complicated by insulin resistance.Methods A total of 100 patients with endometrial atypical hyperplasia and insulin resistance were randomly treated with megestrol acetate dispersible tablets alone(control group,n=50)or in combination with metformin(observation group,n=50).The periodic ovulation rate,single follicular development rate,pregnancy rate,glucolipid metabolism and levels of insulin and sex hormones were measured after treatment for 3 and 6 months.Results Compared with control group,the periodic ovulation rate,single follicular development rate and pregnancy rate were increased and the glucolipid metabolism and levels of insulin and sex hormones were improved in observation group after treatment(P<0.05).Conclusion The combined treatment with metformin and progesterone can effectively increase pregnancy rate and improve insulin level in patients with endometrial atypical hyperplasia complicated by insulin resistance.
