摘  要:目的研究铅作业人员机体微量元素水平、肾功能水平的变化。方法选取某蓄电池厂686例铅作业人员作为观察组,另选取非铅接触作业人员524例作为对照组,对比2组血清微量元素(铅、锌、铜、铁、硒)及肾功能各指标[肌酐(Scr)、尿素氮(BUN)、尿糖、β2微球蛋白(β2-MG)、N-乙酰-β-D-氨基葡萄糖苷酶(NAG)、视黄醇蛋白(URP)]水平,并分析血清铅水平与肾功能水平及其他微量元素间的相关性。结果观察组血清Scr、BUN、尿糖、β2-MG、NAG、URP水平均高于对照组,差异有统计学意义(P<0.05)。观察组血清铅水平高于对照组,血清锌、铜、铁、硒水平低于对照组,差异有统计学意义(P<0.05)。经Spearman相关分析显示,血清Scr、BUN、尿糖、β2-MG、URP水平与血清铅水平呈明显正相关(r>0,P<0.05),血清锌、铁、硒水平与血清铅水平呈明显负相关(r<0,P<0.05)。结论铅作业人员机体血清铅水平明显升高,会造成肾功能水平降低及其他微量元素水平变化。Objective To investigate the changes of trace elements and renal function in lead workers.Methods Totally 686 lead workers in a battery factory were selected as observation group and 524 non-lead exposure workers as control group.Serum trace elements(serum lead,serum zinc,serum copper,serum iron,serum selenium),and renal function indicators [serum creatinine(Scr),urea nitrogen(BUN),urinary glucose,β2-microglobulin(β2-MG),N-acetyl-β-D-glucosaminidase(NAG),retinol(URP)]levels were detected and compared between the two groups,and analyzed the correlation between serum lead levels,renal function levels and other trace elements.Results The levels of Scr,BUN,urine glucose,β2-MG,NAG and URP in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).The level of serum lead in the observation group was higher than that in the control group,while the levels of serum zinc,copper,iron and selenium in the observation group were lower than those in the control group,the difference was statistically significant.(P<0.05).Spearman correlation analysis showed that serum levels of Scr,BUN,urine glucose,β2-MG and URP were positively correlated with serum lead levels(r>0,P<0.05),and serum zinc,iron and selenium levels were negatively correlated with serum lead levels(r<0,P<0.05).Conclusion The serum lead of lead workers can be significantly increased,which can cause the decrease of renal function and other trace elements levels.
