舒目散熏眼对白内障术后泪膜稳定性的影响

摘　　要：目的:观察舒目散对老年性白内障术后患者泪膜稳定性的作用。方法:随机选取接受白内障超声乳化吸除术联合人工晶体植入术符合条件的单纯老年性白内障病例97例,随机分为舒目散熏眼组32例(32只眼),开水热气熏眼组33例(33只眼),参照组32例(32只眼)。所有患者于术前1天,术后1天、10天、30天均进行首次泪膜破裂时间(FTBUT)、平均泪膜破裂时间(ATBUT)、非侵入式泪河高度(NITMH)、睑板腺腺体完整度评分和眼表指数问卷评分检测。结果:参照组术后FTBUT、ATBUT缩短,NITMH变低,眼表指数问卷评分下降,具有统计学意义(P<0.05);参照组和开水热气熏眼组分别与舒目散熏眼组相比,参照组患者与开水热气熏眼组患者相比都显示FTBUT、ATBUT缩短,NITMH变低,干眼症状评分下降,差异均具有统计学意义(P均<0.05)。结论:通过Keratograph眼表综合分析仪客观、精确评估白内障超声乳化术对患者泪膜稳定性有一定的破坏性,舒目散熏眼能改善白内障术后泪膜的稳定性。Objective: To observe the effect of Shumu san on the stability of tear film after cataract surgery in the aged patients with Keratograph analyzer. Methods: A total of 97 cases of senile cataract patients with the conditions of phacoemulsification and intraocular lens implantation were randomly divided into Shumu san fumigating group(32 eyes),water steam fumigating group(33 eyes),and the control group(32 eyes). First time tear break time(FTBUT),average tear break up time(ATBUT),non-invasive tear meniscus height(NITMH),meibomian gland scores and ocular surface disease index(OSDI) was examined 1 d before the treatment,1 d,10 d,30 d after the treatment. Results: In the control group,FTBUT and ATBUT were shortened,NITMH became lower and score of OSDI were decreased after the surgery(P < 0. 05); of which there were significant differences among the three groups(P < 0. 05). Conclusion: Phacoemulsification can damage the stability of tear film to some extent,which can be improved by steam fumigation of Shumu san by Keratograph analysis.
